Utrogestan fatigue
« on: October 04, 2025, 02:47:59 PM »

Hi meno friends! I’m 48 years old and quite new to HRT life. I started estrogel and utrogestan (100 mg vaginally in eve, continually) about five months ago. My last period was 10 months ago so I guess I’m almost classified as post-menopausal. The estrogel has helped with the hot flashes and sleep etc. but I generally feel incredibly sleepy / drowsy the whole time and have a theory that it’s the utrogestan. Has anyone experienced similar and found a progesterone alternative eg cyclogest less drowsy-making? Or I’ve also read on here that some people make a pin prick in the capsule and let some of the liquid out to reduce the dose. But is less than 100 mg safe / protective for the womb etc? V grateful for your advice! Thanks so much,

I have recently started taking micronised progesterone vaginal and feel very sleepy in the day. I'm on 100mcg patch and 100g progesterone although I did increase to two. I previously had a mirena in for three years and had it removed and then the new regime triggered a bleed which has stopped now but I do feel very very tired and my body is boiling. My estrogen is 206pmol so maybe that's too low. I'm fed up with it all. I hope you get some replies that feed in and help. Love this forum.

I get this too and it also disrupted my sleep, it seems to be a fairly common side effect. You could try taking it earlier, I found 10-11am was best but still far from perfect. I stayed on sequi (6 week cycle) when they wanted me on conti so I had more time off it. I don't get it with norethisterone (either oral or in evorel conti patches). I now use a mirena which is great. It isn't safe to use less unless you arrange your own scans to check there's no build up.

I was on 100mg Utrogestan daily taken vaginally and had the same fatigue. My GP has recently changed brands so I now have Zentiva and have noticed that I don't have any fatigue at all. On the downside, I am not sleeping as well as I did on the Utro so I suppose it's swings and roundabouts.

Lots of drowsiness over here on Cyclogest. It's better for me than Utrogestan which caused low mood as well as the drowsiness.

Hi, sorry you're feeling like this. If it helps...me too! I take 100mg Utrogestan orally and the fatigue is horrendous. I cannot get moving and my mood is seriously low. Setbacks I would normally brush off feel overwhelming. I spoke to doctor about it and she suggested moving to 200mg for half the month instead; I couldn't get out of bed. I am speaking to her again today about changing to something else. She previously suggested taking it vaginally to prevent it going through the liver but I see on here that others still get fatigue taking it that way. Wondering if asking to try Zetiva might help.
