Perimenopause, fibroids and continuous hrt

Hi!
Just wondered if anyone is in the same position as me. I am 48 and perimenopausal. I have a large fibroid (approx 10cm diameter when last scanned) - awaiting ablation treatment. I started on Evorel Conti 2 months ago.  The nurse at my GP practice recommended continuous rather than sequi so there was less risk of the fibroid growing. I’ve since read that guidelines advise sequi for my age and because I’m still bleeding. I seem to be getting on with the Evorel conti and my fatigue and leg aches have improved. However, I am still bleeding (unsure if this is period or fibroid). My nurse says we will review at 6 months if there is still breakthrough bleeding. Any thoughts/advice gratefully received….

Sounds pretty standard. I’m 44 and on continuous due to endo….

If you get problematic bleeding going on past 6 months, you might be offered the Mirena coil - which tends to totally stop all bleeding and will also be the progesterone for your HRT. So then you just need the estrogen. If you don't want the coil, you could ask to try a different progesterone - like utrogestan (body identical progesterone) and an estrogen-only patch, continuously. Or you could ask about adding in the desogestrel POP if that doesn't work. So - there are options!

As you still have your own cycle it's likely you'll continue to get breakthrough bleeding. A mirena will usually stop the bleeding even when you're not meno. Oestrogen can make fibroids grow.
