Hi ladies
I’m using 3 pumps of oestrogel and uterogestan vaginally every other evening as prescribed.  Spoke to meno doc on 23 rd and all was ok. We were going to increase gel but I started bleeding and cramps that evening and it’s been ongoing. I started my adrofemme on Xmas day as planned. Today bleeding is more like heavy period and hot water bottle!! I’m 56 and not had period for 9 years. Was on tibilone for 8 years and this has been a recent change. Not sure if I should be taking more or less of something. Any ideas until I can contact. Really can’t be doing with this bleeding after all these years!!

I use 2 pumps of oestrogel and utrogestan vaginally every other night to ,this is my first time using hrt and have been on this regime for 9 weeks.
I have had  some spotting and then in week 7 bleeding for 3 days which was like having periods again ( had not one for a year before starting hrt ) and had very painful cramps ( it really felt like when i was much younger and going through a painful cycle).
I have researched this and the info i have found is that utrogestan ( and i guess some other progesterone) will cause bleeding in the settling period usually in the first 3-6 months although i read too that for some bleeding can occur up to 1 year and should not cause medical concern.
very annoying and uncomfortable side effect though , i have just recovered from unbearable hot flushes snd palpitations and now this….
I spoke to my Gp last week as although i was expecting some bleeding i really didn’t expect the cramps/ pain as this bleeding isnot a “real” period ,and she confirmed that is definitely the progesterone side of hrt that cause that and said that the pain is a consequence of the bleeding.
Unfortunately although she is the only female gp in my surgery that is supposed to treat and have more knowledge of hrt she is not very knowledgeable and she almost send me for a scan , i then told her that the reason i had called her was to check about the pain as there’s plenty of info on utrogestan and how common is bleeding in hrt initially and we decided to wait and see if things settle down before reviewing.
May i ask did you start straight away with utrogestan vaginally and only every other night or initially u were doing ever night vaginally ?
I wonder if the amount of utrogestan has an effect on the bleeding ….

Since we are using the same regime i would be very happy to keep in touch to “compare” and see how this ssme regime goes , please feel free to direct message me should you wish .
In the meantime be assured that bleeding early on seem a “common “ problem .

You can get bleeding for the first 6 months on a new conti regime but they also say any post menopausal bleeding should be investigated so I would contact your gp in case there's another reason for it. According to the manufacturer the conti dose is 100mg every day regardless of delivery method. I believe newson health have tested it at medium doses of oestrogen but not at high levels. If there's been a build up of your lining perhaps you should use it every day, particularly if you go to 4 pumps.

I always thought bleeding was linked to a build up of the womb lining? If the  Oestrogen isn’t effectively opposed by the progesterone, then you get a bleed?

Oestrogen is renound for stimulating the womb. Pain and bleeding are common side effects of HRT I think.

PMB after 6 months, definitely needs checking via a scan.

Hello 
I'm on the same regime: 1 pump Estrogel morning & night and 100mg Utrogestan daily. After 10 months, I have been upped to 200mg to try & stop the bleeding - gp referred me to gnynae after 6months. I've had scans, shows thickening and awaiting biopsy results - hopefully it is down to the Estrogel & the extra Utrogestan will sort it out.
Got to say, after 3 years period free, this has been horrific - bleeding every couple of weeks for 10+days, pain like I've never had & grumpy, very grumpy  . On the other hand the sleep is helpful!
The only reason I've not stopped taking HRT is: it's helped the sleep/night sweats which were crippling! I'm just 2 weeks into the new 200mg Utrogestan so will gut it out a bit longer see if it settles.
Definitely talk to GP if the bleeding doesn't settle, hope you find answers!

Thanks for all your replies. I don’t know how to reply sfifties individually. Will work out eventually. I was started on a tablet orally every night but I’m progesterone sensitive so was said to take vaginally  every other night. Weird as today cramps and bleeding easier after using progesterone last night. As you says sfifties it was like a proper period from many years ago that I don’t want to have. I have emailed the doctor to ask what I’ve to do. I was initially going up to 4 pumps then using vaginallY every night but don’t want to start that until I know this is settled. Only been on androfemme 4 days. I await being a new woman!! I’m happy sfifties if you contact me if you know how!! Hope you had good Xmas. I was on sofa with hot water bottle on my tummy!! Took me bank!!🙄

Thanks for your replies ladies. I’m trying to reply to a lady that messaged me but I don’t seem to have that option. So sorry but will investigate when I’m not working.
I’m still cramping and bleeding on and off. Awaiting to hear from clinic once they open as don’t know wether to increase uterogestan? It’s all a learning curve. It’s alien as all the years I was on tibilone even if I’d forgot to take for few days I had no bleeding what so ever. I just didn’t get hot flushes with tibilone but still had all my other symptoms which just accepted for 8 years. But kinda feel back to square one !! Will let you know how the testosterone goes.

it’s unsettling when a new regime causes problems especially after no bleeding for so long previously on a different one .

My experience and understanding of other ladies experiences here is that before changing doses usually Specialists  or Gp wants to allow 3 months adjusting period and settling of new regime in your case utrogestan every other night vaginally and oestrogel  before reassessing ( not sure how long you have been on this regime?).

Since you are followed privately it’s reassuring to know that u will receive a reply soon hopefully in the next few days and given your are post menopausal and have used hrt before your Consultant might have a different approach and change things before the 3 months .
 I have read some posts here where ladies have been prescribed more utrogestan to help with bleeding especially on higher doses of oestrogel .
I often think that we know how we feel and should follow our instinct but the more time goes the more i realise how complex menopause and treatment of it is that in your case a Menopause Specialist will give you the best advice , but if you are impatient like me it’s hard to wait !
In the meantime i hope the cramping is better ,you can take some painkillers as it must be hard to work if unwell.
