Body aches and HRT. Too little or too much

Hi

I'm hoping someone can help me.  I'm 46 and Peri. I also have Fibromyalgia which started in my 30's.  My journey with HRT started about two years ago.  Like so many of us, I had a GP who had no idea about HRT but one day I struck lucky with a Locum and she agreed to treat my symptoms with Ellestte Duet.  I persevered with it for 8 months or so but the progesterone made me feel so depressed.  I was then given Femoston 2/10 but that was the same.  Eventually, I paid to see someone privately through Newsom Health and was told that I like so many other women that end up there was progesterone intolerant.  I was prescribed estrogel 2 pumps per day and Utrogestan vaginally 100mg for 14 days per month. What I was finding was that on the Utrogestan part of my cycle I actually felt so much better than when I wasn't taking it.  After three months of 2 pumps per day of Estrogel I upped it to three to see if it helped anymore but I just felt more tired and sluggish.  After further reading though, because I was on a low dose of Utrogestan, maybe it was too much eastrogen.

I had my follow up appointment with the NHS Gynae Clinic I was told that the Utrogestan was too low and I should be having 200mg.  I told her that I felt better on the part of my cycle when I was taking the 100mg of Utrogestan so she suggested that I take 100mg pd every day.  I was worried about doing that because i'm still having regular periods so I've decided to up it to the 200mg but for 12 days of my cycle.  I did find however, that taking it vaginally was knocking me out so I switched to orally.  I also had my Testosterone level checked as I have NO interest in sex and haven't for some time and was told my level was 0.8.  She wrote me a prescription for Testogel and I've been using it since the beginning of December.  I've had some issues with it in that the first week I used it, it definitely gave me a bit of oomph! but I misunderstood the dosing and used it for seven days instead of five.  After a couple of weeks, I found that my breasts were becoming so painful and my left nipple seemed to be permanently errect? the pain and tenderness spread down to the tissue underneath my breasts too so I stopped taking it for two weeks.  What I am wondering is if it is making my Fibro symptoms worse.  I had a terrible flare that literally incapacitated me for three days.  My fibro has been pretty stable for the past five years so it felt quite unusual.  I am however, insanely sensitive to medications so it could be I need to get used to it. I'm now using it three days per week and I seem to be tollerating that dose better but everything seems to hurt so much more than usual.  I should also add that since upping my dose of Utrogestan for these past two months, my first period was five days early but was a normal for me period.  This month it was a week late and I had terrible period pain the whole time but it wasn't a normal bleed it was old blood and i've been spotting for seven days.  I don't know if i'm reaching menopause or it's the increased Utrogestan.

My problem is I just feel like my body is a concrete weight that I have to drag around all day.  I've completely lost my get up and go.  Every day feels like an endurance test that I just have to get through.  I know I have Fibromyalgia but I also know I wasn't like this before, this isn't my baseline.  I suspect that I haven't got something right with my HRT but I don't know which bit?!  My hair is falling out again which was one of my symptoms before I started HRT.  I've recently had a blood test to rule out thyroid issues and it was ok.  I can't afford to go back for a private consultation and I can't get an appointment with the NHS Gynae for months so I'm hoping that someone on here might have some ideas for me? Please!

Ask for a referral to a menopause clinic.  Often Gyanes don't have enough knowledge about HRT.  If your symptoms were eased on your regime, why change it? 

7 days won't hurt if it's a 1-off. 

Hormone upheaval can certainly upset us overall.  As oestrogen levels drop the body may become dry: inside and out - scalp, skin, vagina, anus, deep in the ears, nostrils; and muscles may become lax = aches and pains, which could explain the fibro problems.

Hi chaciloves I read your post with interest. How are you ? Let me know how you are now! I’m 3 weeks nearly into hrt. X2 pumps of estrogel daily and utrogestan which was prescribed as 200mg but I’m
Taking 10g vaginally instead.  About 10
Days into the start of gel only o noticed I felt a bit perkier and clearer... not so full of dread and doom. And then ... on day 15 I had to add in the utrogestan for 12 days! I’m
On day 7 today. Since taking I’ve felt rubbish!!! So flat most low and the dread And doom Seems back and I just so tired. I have another 5 nights after this eve to go until my utrogestan phase ends and it’s back to gel only! Not sure how it’s gona go!! I was going to increase my gel to 3 pumps tomorrow as I’ve been reading and advised by many knowledgable women that usually it’s the estrogen still too low so an increase often counter poses the negatives of the utrogestan. I let my Meno adviser know and she said it’s a bit early to increase but I can if I want !  (Rubbish short half hearted response and not enough help!!!)  so now I’m going to leave it until I get to 4 weeks which will be another week and few days!!!! I saw your post and we sound alike ! Will also ask for testosterone after 3 months should all be ok!
