Mood swings and anger
« on: April 21, 2020, 07:40:25 AM »

I've just finished my first cycle of HRT and it's been so good my mood has been back to normal and I've felt like my old self except I've just finished 12 days 100 Utro and my mood has dropped severely. Anger, crying Constantly anxiety falling out with my family and I've still not had a bleed. Today is my 4th day without Utro.
I was on 2 pumps gel and increased to 3 during Utro as my mood dropped but it was manageable. I also take a progesterone only pill & I have been Peri for about 4/5yrs I'm 48
Any advice is appreciated as I feel so sad after feeling so good at the beginning of my hrt journey

Hi Polpringle. Sorry to hear about mood swings etc. First thing is that these feelings like normal pms are to be expected for a couple of days or say after stopping the utrogestan part of the cycle. It is known as progesterone withdrawal. Sometimes gives me a migraine and usually feel like s*** for a couple of days (varies a bit with different cycles). The progesterone withdrawal effects will happen whether or not you have a bleed as it's to do with the physiological changes in your body that occur with and without progesterone. Once it's gone I usually feel great ( as well as can be expected anyway ::) until the next utro cycle! The other things is if you are taking the mini pill too then that's a lot of progesterone in your body when you are on the utro phase. There are alternatives such as one of the COC pills designed for peri-menopause (QLAIRA or ZOELY), or the Mirena coil if you want to continue with the gel. That way you wouldn't overload your body with too much prog and still get contraception. Which mini pill do you take?

Thank you for your reply Hurdity.
I was considering stopping the POP as I don't need contraception do you think that might make things a little easier next cycle?
I was expecting to feel a bit crappy but not like this I just want to start my bleed so I feel a bit better.
I felt so good at the beginning it was a shock to drop so dramatically

I'm in late peri on gel and sequi utro. I find the cycles where I've ovulated I am much worse on the utro phase which I put down to too much progesterone. The cycles where I don't ovulate are pretty much OK. I think there's a good chance without your progesterone pill you feel better.

hi

generally for 12 days on your progesterone phase you would need 200mg utrogestan at the moment you take 100 i imagine because you have contraceptive pill to you would need to check with tour gp if you are going to stop POP whether 100mg would be sufficient to protect your uterus x is it the estrogel gel u are on or sandrena ?

I'm on the estrogel and was told as I take the Utro Vaginally 100 should be enough so long as I get a sufficient bleed. But it will stay under review
it's a meno specialist I see rather than a GP as I have some other health issues & she can tailor my hrt to my needs more

What does she say about your pill? 200mg is the normal dose of utro so it might be worth checking you can stay on 100mg if you stop the pill.

She said I can either stop it or stay on it as it's a low dose. I chose to stay on it for a while as I wanted to see how it went.
I know that 200 is the licensed dose & that under supervision by a specialist other doses are preferable to individuals. So I trust her advise

she also assured me that busy using the Utro vaginally more is absorbed exactly where it's needed so a lower dose can be used with women who are progesterone intolerant.

Try eating every 3 hours 24/7 for 3 months, it took me that long to balance those mood swings.  That includes in the night if one goes across the landing to the bathroom, have a biscuit to hand.
