Bleeding on continual Hrt
« on: July 12, 2019, 12:13:13 PM »

Hi there ,I'm 56 5/6 years post menopause. Started taking taking 2 pumps of oestrogel and 100mg utrogestan 2 months ago . I was s happy sweats stopped immediately and fantastic sleep, which u haven't had for years. Everyone noticed the change and how happy I was ;D. Then 2 weeks ago I started to bleed and haven't stopped since. At first it was pinkish, then a brown discharge and then just like a very light period. I can't see my doctor for another week. Is this normal . Everything was great but now I feel so low. Why does this happen. :'(

Hi Bibby

This exactly what happened to me. I had two scans and a hysteroscopy, and they concluded that it was endometrial atrophy, as my lining was so thin. Apparently this was hormonal, and is basically when the balance between oestrogen and progesterone is not quite right.

You should of course get it checked out, but it's very common, so please try not to panic too much ( easier said than done, o know)

It is quite normal to get some breakthrough bleeding or spotting in the first 6 months on any continuous HRT regime - this information should be in the leaflet.  If you are post menopause and starting from a very low hormone baseline then your body will be ‘waking up' with the hormones stimulating everything again.  On a continuous HRT regime, using progesterone everyday is supposed to prevent teh womb lining from building up but it can take time for the body to adjust to the hormones. If bleeding and spotting continues beyond six months then investigations are necessary.
Utrogestan is not as strong as some of the other progesterones so sometimes the womb lining can build up and this will cause the bleeding. Many women using Utrogestan stick to a sequential regime, so 10-12 days of Utrogestan each month but they use the capsules vaginally ( like a pessary pushed up into the vagina) and they then get a withdrawal bleed after they finish these 10-12 days which will be shedding the lining.  Using Utrogestan vaginally is not licensed here in the UK but it is in many other countries round the world and especially Europe. Using Utrogestan vaginally on a continuous basis (so every day) could cause irritation of the intimate area and also interfere with intimate relations with your partner.
I personally didn't get on with Utrogestan but did very well with the Mirena - we are all different and it can be trial and error before we find what works best.  It can be necessary to try a different progesterone type e.g. Provera or have a Mirena fitted.
Don't be down about this - keep going and see if things settle over the next 2-4 months. Enjoy feeling better.  DG x
