Hi all,

I am almost 52, have been 2 years without a period and was put on Evorel 75 patches and Uterogestan 100mg daily day 1 to 25 (every 28 days).  I mostly feel really well on this regime.

I was wondering if there is any reason why the Uterogestan can't be taken continuously without the 3 day break?

I ask this because I tend to get mood swings when I come off the Uterogestan- even though it's only for 3 days.  I seem to be super sensitive to any hormonal fluctuations. I also have PCOS which I suspect doesn't help!

I'm a bit anxious as we're having the in-laws to stay at Christmas time which coincides with the 3 day break from Uterogestan and I'd rather avoid any outbreak of WW3 over the festive season (father-in-law has tendency to wind me up over politics).

I'd be grateful for advice...

The licensed dose is 100 mg days 1-25 out of a 28 day cycle. As far as I know this hasn't changed - Dotty can you let us have any info on this please as I wasn't aware of this?  In practice though doctors tend to give the alternative as continuous use. The 3 day break is to allow a bleed, because  the lining can build up depending on absorption and oestrogen dose. The actual product info (summary of product characteristics) - still gives days 1-25 and says that bleeding should be less on this dose (than the 12 days x 200mg). My patient info leaflet makes no mention of 100 mg at all which is very strange considering the 200 mg single capsule was withdrawn for HRT!!!!

If you don't get a bleed on the 3 day break and haven't done so for two years then sounds like it shouild be OK for you - but do mention to your doc at next visit and be vigilant about any spotting or bleeding.

I know what you mean about the withdrawal - I hate actually taking the prog but also hate the withdrawal - many of us suffer double whammy on this, but at least if it suits you while taking it you can hopefully avoid the withdrawal symptoms.

Dr Renee Hoenderkamp explains how to use Utrogestan . She states that:

Bleeding regimens:
Oestrogen daily
Utrogestan 200mg days 15-26

Non Bleed regimen:
Oestrogen daily
Utrogestan 100mg every day (no break)

If the break is for the same reason there was a 7 day combined pill break, then that has now been discounted as was seen on The Truth about the Pill a couple of weeks back.

The pill break was invented to allow a withdrawal bleed because:
A) it was ‘assumed' women liked to have a bleed (for confirmation they weren't pregnant)
B) to satisfy the Church who would only support the pill if there was a monthly bleed (for religious reasons).

The doctors on the programme said there was never any scientific reason for the break and that this was now the new advice for GPs to prescribe it back to back. They mentioned that the leaflets would still say to take a break and it wasn't clear if they would ever change or if they were in the process of changing. It would be a big manufacturing change as some have sugar pills for pill breaks too.
