Kliovance or Sertaline?

Ok so I started taking Kliovance about January this year after being on Elleste duo conti for 3 months, thought the Elleste was causing depression and anxiety so switched to the Kliovance as hoped the lower dose would help. Felt ok for a while then the anxiety returned so weaned myself off it, again felt ok for a few weeks but got another bout of depression so went back on it again!
Have been on it for 2 months now and I'm back to square 1.
Early morning waking, feeling anxious, days of feeling really low and to top it off loosing hair at the temples and sides of my head.
All this has been going on for the last month or two and I'm sick of it haven't felt happy for ages and just try and get through each day the best I can.
I want to come off the HRT altogether as the only thing it works for is the hot flushes.
I just don't know what to do anymore I get anxiety when I'm on it and off it so I'm guessing it makes no difference whether I take it or not.
Doc wants to try me on sertaline I'm not keen as I've never suffered with anxiety and depression before and can't see how it would work as I'm 99% sure it's hormones that's causing it.
I'm 55 had my last period on my 50th birthday so I would say I'm 5 years into menopause but all these symptoms only started in the last 6 months.
I've been prescribed some Valium for the anxiety but it only works for a short time and doesn't help when the low mood strikes.
The hair loss is making me more anxious as well so I really have no idea what to do, carry on with HRT or go on Sertaline?
Any advice greatly received.

Where to begin! Sounds like you would benefit from trying a different HRT from what you say.

If you have never suffered anxiety or depression before then you should not be prescribed sertraline in place of HRT and probably should not have been given Valium if your anxiety is hormonal ( or due to the progestogenic side effects!).

Some women only begin to experience menopausal symptoms - of which anxiety and low mood is one (from low oestrogen), when they reach menopause, and for some like in your case - the symptoms of low oestrogen do not become apparent until some years later. Oestrogen levels drop on average for two years following the last period so often women experience worsening (or initiation) of symptoms at this stage.

The fact that your flushes have ceased indicates that you are probably on the right dose of HRT (oestrogen). Unfortunately Kliovance and Elleste Duet conti contain the same synthetic progestogen - norethisterone - which is androgenic ( and can cause symptoms such as hair loss) and also because you are taking it all the time can result in continuous low-grade progestogenic side effects including headaches, anxiety etc.

You options are to return to a cycle - so that you only have to take progestogens for part of the month - but you would need to have a withdrawal bleed), or to change progestogen and see if this makes a difference. It could also be that your oestrogen dose is not high enough.

You could try a transdermal HRT - such as Evorel or Femseven sequi ( Evorel contains the same progestogen as you are taking now - so probably best not due to hair loss) which would give a monthly withdrawal bleed, or you could have a separate oestrogen patch and progestogen ( either progesterone = utrogestan, or Provera) - continuously or cyclically.

The other alternative if you prefer to stay with a tablet is Femoston - the progestogen is not androgenic and is better tolerated. This comes as continuous combined preparations ( 0.5 mg or 1 mg) or cyclical (1 mg or 2 mg).

All the different types are listed under Treatments/HRT preparations (top tabs pale green banner).

Do not give up yet - there is surely something out there that will suit you! I started HRT just a bit younger than you ( almost 54) and I am still happily taking it 10 years later.

There is also a site nomorepanic which might contain some helpful tips about coping with anxiety that do not involve taking meds.

Thank you for answering.
You've gave me a lot to think about, I think I'd like to try the Femiston and shall maybe see a different doctor the one I have at the moment is lovely but didn't seem to want to discuss alternative HRT even when I told her I thought it might be the progesterone in the Kliovance that was causing the anxiety and low mood.
I'm not very good at getting my point across at the doctors as I forget what I want to say and am worried about wasting thier time when there's people with serious illnesses and here I am moaning about my menopause!!!
I shall write down some points in your reply and take them with me to the Docs. I'll carry on with the Kilovance for another couple of weeks as that will take me up to being back on it 3 months and unless I feel drastically better by then I will ask to try an alternative HRT as I'm worried about side effects from anti depressants and as my anxiety and low mood seem to be hormonal can't see how they'd help.

Hi Marie62- I'm not sure that keeping going with the Kliovance is going to help. Maybe best to go back to the docs and ask for Femeston sooner rather than later. I have a progesterone intolerance but all I can say is that norethisterone makes me feel truly awful and when they switched me from cyclical to continuous, I was off the scale mood wise, headaches, crying,  the full monty.

I agree with Hurdity- you may be able to tolerate another type better and you may decide that you want to only take it for part of the month. Your GP  should be offering HRT rather than ADs as a first line of defence. Try and get the hormones under control first and foremost and you may find that you don't need them.

Definitely write down the points you want to make and do lots of reading before your appointment! There is a lot of info on the main part of this website and some of the news items are really good - especially the ones that review HRT and recommendations.

Kliovance caused me to have panic attacks/severe anxiety.

I'm currently trialling Angeliq plue Testogel. Both femoston and Angeliq have a kinder Progesterone so may be better for you.

I think stopping the Kliovance is the way to go. I'm going on holiday on Saturday for 2 weeks so won't see a doctor until I get back, should I maybe just stop the Kliovance now or wait until I get back in two weeks time? anxiety is quite bad just now but I'm ok if I keep myself busy and eat regularly, the hair loss is worrying me so just feel like stopping it right now.

I wouldn't just stop. Try halving your tablet and if you feel worse go back onto a full tablet.

That's what I'll do the tablets are easy to cut in half, hope the anxiety doesn't get worse but it's a low dose pill anyway and last time I came off them didn't notice much difference in my mood, but was only off them for about 3 weeks then the doc told me to go back on them, wish I'd just tried to stay off them in the first place!!

I agree I wouldn't just stop suddenly especially as you are going on holiday - it might make your symptoms temporarily worse! If you haven't had any bleeding or spotting while taking the Kliovance, it is just possible you might get a little if you halve the tablet - due to reduction in the progestogen and if there is any womb lining to shed. This being the case as a precaution I would take some sanitary protection with you on holiday just in case!

Hope you enjoy your hols anyway - try to relax and forget all about your worries and all things menopause! :)

I've taken half a tablet today and will continue to do so throughout my holiday and will pack supplies just in case of a bleed!!
Anxiety pretty bad today but that might be due to worrying about feeling rubbish while I'm away, I will try my best to ignore it and hopefully a change of scene will do me good.
