Hi ladies!

Off to see Prof. Lumsden again tomorrow after waiting 5 months(!) this time and disaster on Tibolone. Feel a bit more confident but mulling over my options of what to try next. Here's my thoughts:

Continue with estrogel. I hate that I obviously need split dosing  - It gets me down to be honest.  Also skin is already showing signs of irritation on this again.... >:(

Titrate through doses of Evorel  (BMS not recommending Estradot at present due to supply issues) with thigh application due to problems absorbing whilst on Estradot when applied to tummy. Muscle circulates hormones better. No guarantees i''ll absorb better this way but is it worth a try?

Oral estradiol - I'm assuming Elleste is favourite here on MM? Concerned about dose 'peaks and troughs' due to first bypass of liver.

Premarin (again) but had problems with dizziness and nausea last time. No guarantees this will resolve.

I will be requesting testosterone as there is no reason for me not to have this immediately to start as I've been oopherectomised, and Prof. Studd has always prescribed this immediately alongside estrogen to women in my position. I'll go into this at a later date as to why we're different.....(Prof. Studd was happy enough to write me a prescription for this immediately last year which sadly and foolishly I never filled).

Anyone have any thoughts/ideas for me please? I'd very much appreciate it. Thank you! xxxxxx

I'm really stumped for ideas but just wanted to wish you luck

If I were in your shoes I'd either try the regime you felt your best on.... Premarin? Or try a new route.... oral oestrogen and beg for T gel xxxx

No real insight into what you can do really,however you know your body/system the best so go with what you think is the lesser of the evils.
Good luck with tomorrow and let us know how you get on and what you decide.
It will get better in time and you will find something that suits and helps you personally whether that be hrt,ads,a combination of them both of something else entirely. Wishing you well.

Tempest, from what you have said, it sounds like the oestrogen delivery method is the problem and you don't get on with hormone fluctuations.  Have you considered having an oestrogen implant?  It could be the answer once you know how much oestrogen you need to feel good. 

Why not ask Professor Lumsden how an implant would work and how often you would need to have it changed?  The gel (using a split dose) would be a good way to find out how much oestrogen you need because it is very flexible as long as you can cope with the skin irritation. 

I would also try testosterone as soon as possible.

Thank you, Mary G! Yes, this is definitely one of the problems I've identified with fluctuations. I have been offered implants but as you rightly say, I have to establish first at which level I feel best so need to find a delivery method of HRT in the meantime that would enable me to do so.

Implants are available in 25mcg and 50mcg - rarely are implants offered over 100mcg due to the risk of tachyphylaxis. The implants are replaced every 6 months, with thorough testing of levels prior to replacement and levels are allowed to drop before then, usually not drastically but supplementation with patches or gel would be needed for a very few weeks toward the end of the implant's life.

I shall definitely be asking about Testosterone after my telephone discussion with Diane Danzbrink today! xxxxx

Tempest, if you get on well with 2 pumps of gel, the 50mcg implant sounds promising and might be the way to go.
