What is normal endometrial thickness?

Hi everyone,

I have been on HRT for 7 months and just went for a scan today to see how my womb lining is faring.

It is day 4 of my cycle and my periods have been light so I was concerned the progesterone may not be doing it's job.

I am 49 YO, was premenopausal before I started taking HRT (IE: regular periods coming roughly every 23-25 days per month).

I have my photos and from what I can read (haven't seen doctor yet) it in 9mm thick.

Is this abnormal???

Everywhere I look I get a different story!

I'm freaking out a bit actually.

Anybody who could help I would be very grateful

Hi soniad

Anything 4 or under is acceptable I believe, someone please correct me if that's wrong.
Mine is 10 and no amount of progesterone will shift it in fact it just gets thicker
However this doesn't mean you will experience the same. Go and see your DR and talk it through with them. In my experience I think you may have a small pipelle sample taken, a hysteroscopy and scans to keep an eye on your lining
Are you taking progesterone as part of your hrt ?

Hi Soniad,
I had a similar issue a month ago and was so worried I posted here my concerns . In short, I went for the regular 6-month gynae review after I had stopped HRT for about 2months. I am still peri, was on Femoston continuous HRT up until November last year when I decided to stop and see how I will feel without the pills.

The gynae measured my lining at 13mm on day 18 of cycle. He said that was a lot and prescribed a separate progesterone to take day 16-25 every month. I then read that lining of 10-16mm is quite normal in late lutheal phase. The next day I went to see another specialist who said lets wait for your period to arrive and we'll measure the lining then.

Sure thing my period came on Day 24th. My second measurement was on day 7 of new cycle. It was 9mm and the uterus and the lining all looked very healthy according to the doc. I also had, on day 7, a large folicule already formed in one ovary and ready to burst so this meant  I was very close to ovulation.

The doctor said since I am peri, my cycles might have shortened so when I ovulate, I ovulate early, on Day 10 perhaps, periods coming 20-24 days. This means that on day 7 uterus lining of 9mm is very normal and I shouldn't take anything for now. I am due for another scan in 3months.

So I want to say that you should not worry. Your cycles might have shorten as you say, so you ovulate earlier and your uterus builds up earlier.

In addition - I was just wondering what dose/type of HRT you are on and how much progesterone? There is usually more concern if there is abnormal bleeding as well as thick endometrium - because that is due to it breaking away sporadically. Apparently there is more likely to be some abnormality if there is bleeding and a thick lining rather than thick lining per se - notwithstanding that thickness varies thoroughout the cycle as Milamam points out.

I am so relieved to hear your story, there is literally no info on the internet in relation to women in peri who are taking HRT and what an acceptable lining thickness is. I'm so grateful for your response. :)

Walking the dog, thank you for your reply - I'm taking Utrogestan as part of my HRT. Are you taking HRT and are you postmenopause?

Hurdity, thanks for your reply too. I was hoping to hear from you.

I am taking 43.5 mcg estradot patch (patch snipper) and 100mg Utrogestan vagianally. I started taking it 15 days of the month but my periods were a bit light so I thought if I reduced to 10 days per month they may get heavier.

The flow is heavy on day one and two then this last period it petered out to just spotting on day 3 and 4. Hence the decision to get a scan done. I just wanted to see what was going on.

I have had absolutely no problem with breakthrough bleeding and no spotting between periods.


It's just that on day 4 (period has ended by this day), I would have thought the lining should be much thinner than 9mm.

I'll definitely go back to 15 days, I was even thinking I should maybe take the prescribed dose of 200mg for 15 days.

Any thoughts?

Will increasing the progesterone help to thin the lining a bit?

I can't help with how to achieve a thinner lining, but to put your mind at ease, the cancer risk is very slim even with the thickened lining, it's just significantly higher than the practically nil risk with thin lining, if that makes sense. So yes, if it's thick it's worthy if concern, but shouldn't cause excessive worry.
