HRT application route / location etc useful website link

All, I found a useful site which discusses route implications / pros and cons of different application routes in some detail here :

Its a blog by someone who's had surgical menopause but it covers oral, dermal, vaginal / rectal, gel, cream etc application. I did find another paper (now lost ref unfortunately) which suggested that further away from the uterus the application was the less dose would be delivered to uterus, but that would be primarily relevant for progesterone - for oestrogen depends where the receptors are....and on the other hand that soft / fatty tissue would hold the dose better.....don't be offended!

Hope that's useful - academic knowledge only on my part.

Thanks so much for this. The Surmeno blog is really very informative.

I stumbled on it the other day and got thoroughly confused by the section on measuring estrogen blood levels - no fault of surmeno - it is a confusing issue! I have some questions for the wise ladies here on that which I will compose and post on a thread in a couple of days.

Thanks KatyB - some of what she says gives the lowdown on some of the effects of the different absoprtion - in layperson's terms which is helpful!

However it's an American viewpoint - and includes different types of HRT that are prescribed through private practitioners such as sub-lingual and oestrogen creams which are not prescribed or licensed here. Also some of it is perhaps misleading in UK - like application of oestrogen creams to chest. As long as readers bear in mind that the article is from US and is also one person's interpretation of the information rather than the scientific studies - that's great :)

If anyone wants to look into this further I can also post some other links - thanks for starting this KatyB!

i found the link for the effect of different locations of patches - this is the writers summary of the paper she has read - she does include a link to the paper but there is a sign-up required. Hurdity is quite right, its a US site, hence the focus on compounding etc. But given the reports people have given about patches, weight loss, location questions etc I thought it was quite useful link. The quick summary is "As a rule, though, best results are obtained in the outer half of each body half, but not so far "around the edge" that there's a lot of curve to the location and not directly over an area where there is little padding over the bone or over large muscles.

The other location factor is that studies by manufacturers have shown that patch delivery (same patch, same dose, same degree of adhesion, different location) varies by 17-25% depending on whether it's on the front of your body or back. That is, locations on your butt deliver more to your system, from the same patch, than if it were placed on the front of your body."

Hi KatyB,

Thank you for the link and for starting this thread. The patch location/dose/absorption discussion is an ongoing process and this has been very helpful. That blogger has also a forum with thousands of users (but only for ladies under surgical menopause).

Interestingly though - for (the now discontinued) testosterone patches - the reverse was found! I think larger numbers of women need to be studies to get definitive answers as it probably varies according to how much fat we have in the different areas!
