heavy irregular bleeding on Evorel sequi after 3 months

Hello I am new and have trawled the forum for advice, I realise the information may be out there but here goes. I am 50 and peri-meno. I was taking low dose combined pill but changed at 50 as per guidelines. Now on Evorel Sequi- feel amazing mood wise, libido, energy but getting heavy bleeding which after 3 cycles has become a problem. Bleeding was in 2nd two weeks initially but now started in first 2 weeks. Took patch off today and looking at options. Should I perservere longer or change progestogen element. Really fed up as thought I had cracked it !. Thanks in advance

Hi and welcome kratc
The peri stage can be challenging because your own hormones will be fluctuating in the background. You have not been on this HRT for very long but it is a medium dose which might be a bit high at your stage of the menopause - a lower dose is usually advisable at first in peri meno. It can take at least 6 months for the HRT to control your cycle - the bleed should actually come after you have finished the oestrogen/progesterone patches - so in fact you now seem to be getting the bleed at the right time. If you are feeling generally good then I'd just continue with the patches as prescribed and hope that the bleeding settles as your get deeper into the menopause.  If the bleeds are very heavy and prolonged then you may need to try a lower dose HRT as you may be getting too much oestrogen which will build up your womb lining and cause more of a bleed - it is unlikely that a change in progesterone will make a difference in the bleed. You may need to have a scan to see if you have fibroids which could also cause excessive bleeds.
If you would like to reduce and possibly eliminate all bleeding then having a Mirena fitted can be a good option as you can then just add oestrogen as gel or patch and this can be an easy way forward. DG x

I was just wondering how you knew your were perimenopausal if you were on the low dose CCP? Also Evorel seems to cause heavier bleeds than some other HRT types - although that is only anecdotal evidence from reading women's posts on here.

Not sure why you want to change the progestogen element? If it suits you and it's doing its job and you are feeling good otherwise then that's great. However I agree it might be worth having this looked into just to check your uterus. Glad you are feeling better generally :)

Thank you so much for replies think I will stick it out a bit longer, as suggested and see GP re investigations. I went on combined pill as having peri symptoms  and being under 50 and needing contraception,it suited me. I had no contraindications and had reacted badly to progestogen only methods. I probably should have took a hormone break after stopping to see what my symptoms/cycle were like without. Too be honest was too scared to try. Hopefully things will settle but at the moment the bleeding is heavy and painful. Thank you again, its good to know support is out there
