Need low oestrogen HRT , advice please

I have been given permission by my consultant to try HRT, needed his advice as GP too scared to prescribe due to my blood clotting condition.
So he says I should have low oestrogen and I have a feeling I want what you call bio identical, and trans dermal as i have migraines. At that point I well and truly stuck. I am 53 still have irregular periods. I don't know what to ask for. I am very sensitive to any hormone changes so am pretty scared really.

I wonder should I ask to go to a meno clinic? He is a young GP will do anything I ask but obviously will not exactly understand my needs.

Just another thought is there a chance HRT will help me sleep and improve my mood? I don't have sweats, my sleep and mood are my worst things.

Almost all oestrogen now is bio identical unless you get the one from horse urine. So you should have no issue there. If you are using oestrogen only then a gel would be good as you can use a tiny amount.
If you have to use progesterone then Utrogestan is the one you would need. I would suggest using it vaginally.
You could email Dr Currie for advice and then take her reply to your GP and he could prescribe from her recommendations.

Oh and meant to say it can improve your sleep especially the Utrogestan and that on itself may improve your mood.

Honeybun I gather I need progesterone as I have not had a hysterectomy.
Are you saying i would not have any pills to swallow as such?
Is utrogestan oestrogen and progesterone mixed

Yes no pills to swallow. Oestrogen gel rubbed on and Utrogestan as a pessary. Utrogestan is not licenced to be used that way for HRT in this country but it's perfectly acceptable to use.

You need to work out whether you want a cycle or not......and that depends on your age and whether you are peri or post meno.

I am 53, don't have regular cycle as just had 4 months without then got a period, so I am peri. I dont care if I have a cycle if I slept better and was happier. But how do you know when to give it up if it gives a cycle?
Does that effect the amount you have if I am peri? When you say its not licenced how do we get gps to prescribe it then?

It's all very individual I'm afraid what does something for one person does something else for another ! That's why it really is all trial and error. Me personally I've never found hrt helps with my sleep and anxiety that's why some of us use an AD as well. Hope you find a regime to suit you.

dazned I am already on ADs, they may have stopped working but i dare not try to change. however as my mood is not always down I am pretty sure hormones are to blame. As for sleep, that is a mystery, I wake every hour, need to loo, crazy. I wee more in the night than in the day. I dont have any caffeine, hardly drink before bed, have no idea what to do really

Have you kept a mood diary that way you can see if a pattern emerges i.e. is it cyclic? What AD are you on maybe try upping dose a little bit ? Needing the loo more frequently might be the start of VA,maybe local estrogen would help with that.

Sorry it's a bit confusing. GPs will give you Utrogestan to be swallowed but the same pill can be used vaginally....it cuts down on side effects. Utrogestan is used for fertility treatement this way and also widely prescribed in France for use vaginally.

You don't get an applicator , you just sort of stick it up there  ;D. If you didn't like the thought of that you could swallow it. It's just you mention using bio identicals and that is the only one.

As for having a cycle, the general rule of thumb is a year with no periods or over 54. However just to confuse things even more that's not always the case. I've been on a no bleed regime for almost three years now and I'm 54. At your age it's really your personal preference. It really depends how you get on with progesterone and if you can tolerate it all the time which you would need to if you prefer a no bleed regime.

Difficult to explain but I'm sure you get the idea.
