Unexpected bleed - should I be worried?
« on: February 25, 2014, 11:13:15 AM »

Hi

I changed to estrogen patches and progesterone tablets (one a day, no break) just before Christmas.  I haven't had a period for just over 3 years.  I've just started to bleed (not much), I did forget to take my tablet one day over the weekend.  Could it be due to me forgetting my tablet?  I'd done this on my combined tablet sometimes and this didn't happen.  Should I be worred?

Any opinions would be greatfully received

Can't tell you - someone will be along I'm sure, otherwise have a word with a Pharmacist or your Practice Nurse?

I had breakthrough bleeding when I was changed from an HRT where I had a bleed to Tibolone where I didn't (ie it was a continuous HRT)

I also got in a muddle about missing Tibolone tablets and my GP said this could result in breakthrough bleeding. But if it happens again I would certainly go and get it checked out. Or of you are still worried go see your GP.

What sort of progesterone are you taking and what is the dose?

If it's Utrogestan, some women find it harder to absorb, so missing one tablet could bring on a bleed. You could also be bleeding now because you have not absorbed enough of the progesterone over several days ( even though you took them) and most people find they bleed 2-4 days after the last tablet.

Hi, thanks for your replies.  Sarah, I'm taking progesterone micronised 100mg tablets.  I've a feeling I might have missed 2 days, I've had family visiting and my routine has gone out the window, it doesn't help that these tablets don't have days of the week written on which my past medications have.

I was going to have a chat with the doctor at some point anyway, this has prompted me to book earlier than I would have.

Its missing the Estrogen with me that brings on a bleed, just forget one and whammo, thats it, a real nuisance, our bodies become very sensitive to these added hormones...and the dips and climbs...

Strange how we are all so different. That's really unusual Cassie as it's normally the withdrawal of progesterone which kick-starts the womb lining into shedding.

rhinogirl - if you think you missed 2 days of the progesterone then that is the most likely reason. As Taz says - when you withdraw progesterone this triggers a bleed.

However as you are on continuous combined, you shouldn't really get any bleeding when you stop progesterone as it is not designed to induce a cyclical bleed. This means you do have lining to shed, which means the dose of progesterone may not be sufficient to thin your lining, which could be due, as Sarah says, to lack of complete absorption.

What dose of patches are you using and I presume you are taking 100 mg utrogestan?

Yeah, the drop in Progesterone also brings on my bleed, but that is only after 5 days but if I forget to apply the gel for one night, I get a small bleed, its obviously when the oestrogen levels dip...
