Hi,
I have a mirena coil and use oestrogel. These have worked well for me and I’ve been pretty stable for the last 2-3 years. I’ve recently had a horrible crash in my mental health and trying to understand how much might be hormone related. I though the Mirena coil lasted 5 years but have just seen on some websites it says 4 years if used for HRT as the progesterone starts to wear off, though it looks like it has changed to 5 years on some sites. Mine has been in 4.5 years now… wondering if this could be a factor. Anyone have experience of this? Thanks

IUS is licenced for 4 years as endometrial protection but widely accepted by many authorities as being safe for at least 5.

Unofficially there's evidence it is effective beyond this but not enough in terms of numbers to change guidelines across the board.

I would think it very unlikely that your IUS has suddenly turned on you.

The decline in levonorgestrel released is sharpest in the first year and very gradual thereafter.

Of course you could try changing it however most mood related side effects seem to occur when it is new and releasing the most levonorgestrel, vs close to expiry.

If you are someone who feels progesterone is beneficial from a mental health perspective there is also no reason you couldn't have a trial of micronised progesterone whilst you still have the IUS.
