Use of Ovestin
« on: September 19, 2024, 05:54:39 AM »

Hi all,
I am on day 20 of Ovestin and it is working nicely. My bladder is not back to normal but it is much improved. I am also on BiEstro/Progest 5 days of the week to deal with hot flushes - the creams work very well for it.
As for the Ovestin - I went back to my doctor for a script repeat and she told me it is time to start decreasing my dose to every second night.  I am not really comfortable with that just yet and I told her so.  I have seen others comment that they still use Ovestin daily after a prolonged time.
May I have some advice please?  I don't want to go back to those awful bladder symptoms by reducing my dose too early.

U R making progress!   How many boxes were you prescribed, I get 2 each time.

Put her head down the loo and flush the chain.

She's wrong.  What's her worry is the 1st question you should ask?  Too many think that we can overdose on oestrogen.  Some of us need nightly treatment for ages.

Ring the Surgery to ask what she is basing her idea on?   Dr Newson is trying to get the detalls in the leaflet in the box altered because too many medics are sticking to them religiously!

Point the Surgery to this Forum ;-).  Don't wait until your next script is due, begin the battle today - we are behind U !

Sorry, no advice on ovestin.

But you use BiEstro/Progest??? Estradiol/estroll cream??? And a separate progesterone cream?? And you find they work well?

Thanks.

Haha, (me imagining flushing her head down the loo!)
I have 3 tubes in the bathroom - I am taking no chances.  At least she gave me a repeat on my first script, and I have just gotten another script with a repeat.
She gave me warnings of breast cancer and DVT etc.  The usual.
It has only been 19 days, I have no intention of going backwards, to how I was feeling before so I will carry on nightly while it is helping.

And yes, the Emerita brand of BiEstro/Progest is a nice balance for me.  I do find I feel a little agitated and restless about half an hour after the creams, but it settles within 15 mins or so.
I have to use them together in the morning as I use Ovestin at night internally.  The creams seem to have put to rest the anxiety and hot flushes I was suffering with.

Her advice is completely out of date, carry on as you are doing, why would you not if a treatment is working, a treatment that is completely safe.

Thanks, I will. Tonight, for example, I was quite uncomfortable after showering, with my bladder urging a bit.  Half an hour after Ovestin, the symptoms are relieved.  I'm not stressing myself out by wondering whether or not I should use it on a certain night.  I'm looking for relief and whatever will provide it!

These are the latest guidelines from the Position statement for management of GSM (Vaginal Atrophy) Dr Louise Newson and referenced by many others in their field.  NICE guidelines due to be updated in November which will hopefully include appropriate treatment doses for GSM.

"Women should also be advised that the information packaged with vaginal hormone preparations is out of date and factually incorrect. This needs to be changed by the MHRA.
The dose of vaginal oestrogen is very low, for example, using 10mcg oestrogen pessaries regularly for one year is an equivalent dose to just one 1mg of oestradiol HRT tablet.
If symptoms do not improve with vaginal oestrogen, consider increasing the dose, changing preparation (and consider DHEA) or using an additional treatment such as systemic HRT or two local treatments.
The frequency of using these preparations can be increased in women who have persistent symptoms as the doses of these preparations are very low.
Vaginal oestrogens can often improve urinary symptoms, including urinary infections.
Some women use vaginal oestrogen preparations more frequently than the recommended dose to improve symptoms sufficiently, for example Vagifem alternate days instead of twice a week. It is safe to use the treatment in this way as the dose is very low.
Women should be reassured that it is safe for topical oestrogen to continue in the long term (medication should be on a repeat prescription). Women should continue to be reassured about the long- term safety of vaginal oestrogen preparations."

Ask them what exactly they think is going to happen if you continue to use it daily, given that systemic absorption is negligible.

And please share the answer, I'd love to know!

I'm still for putting her head down the loo ........  :whist:

Despite the British Menopause Society apparently letting medics know the ins and outs of HRT  >:(. Something that doesn't appear to be working ?!?!?

Every night when required.  When I get the need2P I swallow 2 Nurofen 3 times a day - I may have said already  :-\

I just came from an appointment with a nurse practitioner.  I told her all about my estrogen adventures.  She said it was wonderful that Ovestin worked for me and to keep using it nightly for 3 months before having a review with the doctor.  Her words were "doctors are so afraid of estrogen - I don't know why!!".  She also said that improvements 20 days in was excellent and that things should only get better.
Fabulous lady :-)

That was exactly my experience. A new surgery, doctor asked why I needed Ovestin (Estriol) and Vagifem because it was rare to need both.
My review came up soon after, I went armed with all the latest guidelines and was ready for a battle but I didn’t have to worry, a fabulous practice nurse that was well up to date, we sat there speaking the same language, so refreshing and so unexpected.
