Estriol and Estradiol
« on: September 02, 2024, 05:34:26 PM »

Are topical Estriol and topical Estradiol the same or is Estradiol a stronger form of estrogen?
Is it safe to use topical Estradiol everyday?
My Gynecologist prescribes Estradiol 1 gram 3x weekly and always tells me not to go over. That seemed to work for me for several years but the last few months not so much. I have started having VA symptoms like never before over the last 3 months. Thanks for any advice.

Estradiol is in pessary form and Estriol is a cream with estradiol being stronger. But its something I have never been bothered about and use both treatments every day to control severe symptoms.

Latest guidelines on usage that you might find helpful from the Position statement for management of GSM

"The dose of vaginal oestrogen is very low, for example, using 10mcg oestrogen pessaries regularly for one year is an equivalent dose to just one 1mg of oestradiol HRT tablet.
If symptoms do not improve with vaginal oestrogen, consider increasing the dose, changing preparation (and consider DHEA) or using an additional treatment such as systemic HRT or two local treatments.
The frequency of using these preparations can be increased in women who have persistent symptoms as the doses of these preparations are very low.
Vaginal oestrogens can often improve urinary symptoms, including urinary infections.
Some women use vaginal oestrogen preparations more frequently than the recommended dose to improve symptoms sufficiently, for example Vagifem alternate days instead of twice a week. It is safe to use the treatment in this way as the dose is very low.
Women should be reassured that it is safe for topical oestrogen to continue in the long term (medication should be on a repeat prescription). Women should continue to be reassured about the long- term safety of vaginal oestrogen preparations."

 :thankyou:

Your Gynae is out of date!  As R many of the UK's medics.  Use as much as is necessary to keep the VA under control, some ladies require nightly as well as moisturisers i.e. 'sylc', 'yes' or estriol.  I have the latter and smear a little on the outer labia/vulva when the area feels dry and itchy.  Occasionally I have to do a 5 night reload when symptoms niggle, usually a sensation below the belly button: 'fullness'.

When I get the need2P feelings I take two nurofen 3 times a day.  So many medics fear that ladies can over dose on oestrogen -------- Quality of Life is important so what ever works to get the condition under control.

If your Gynae insists that you are using 'too much' then I suggest that you get a referral to a dedicated menopause clinic.  Browse round.  Make notes.
