So on the News today again computer use has shown how un-reliable this form of note taking is. Medical negligence I reckon, patients information not being sent to GPs etc. because the discharge letters - on computer - weren't signed off probably so disappeared into a holding, hidden folder.
What's wrong with taking it onto proper pen and paper ............. except that medics are well known for not having the best hand writing ever!
