Progesterone and cramps
« on: May 06, 2020, 10:47:43 AM »

Hi everyone. I'm hoping to get some answers or reassurance.
For the past 2 years I've been going through peri menopause. My doctor is pretty useless. Said I am, then I'm not, then I am going through it. Put me on combined HRT which helped with hot flushes. But as I was still having periods then changed me to oestrogen only for half the month and then to add progesterone for the other half. ( at ?20 a pop!!)
I'm now not having regular periods but after the progesterone patch part of my cycle, I always get terrible cramps. But this time no period!  :'(
The doctor tells me there's no point in me having a blood test whilst I'm on HRT as it won't be an accurate result.?!????!
My moods are all over the place, terrible anxiety....and yes the hot flushes are gone but the cramps are worse in a way.
Is anyone else having similar issues?

Hi Spaceship75

Just wanted to welcome you.   :welcomemm:

I still get cramps occasionally now, and I'm post meno. (5years) .
Certainly, I think progesterone does make it worse.
Maybe you need a higher dose of oestrogen now? 🤷‍♀️
Sorry can't be any more helpful.
Someone will help you soon, I'm sure.

Hi. Thanks for your replies. I was on Estradot 37.5mg ( half month) and took utrogestan 100 pills for other half. Terrible cramps. Now Changed to estradiol 50mg half month and arboreal conti the other half. No bleed but cramps . When I took Estradot  and changed to evorel got bad moods and anxiety.
So fed up with it all. I've cut my patch  down so not having as much oestrogen. Will have to wait and see I guess. But thought I was suppose to have a period when taking progesterone?

Hi

Just to add my two penneth.  I recently switched from oral utrogestan to vaginal every other night (under guidance of my meno specialist).  However, after 3 weeks I started to bleed, and like you had horrible cramps at night.  Because of this I've had to go back to oral route.  I, like Jillydoll,  am post meno.

It is strange that you've had no period on sequi regime, have you asked your Dr about it?

Not all women have a bleed even on sequential.

This is from a BMJ paper.

jfprhc.bmj.com/content/28/4/182.full.pdf

"On sequential HRT 85% women get a regular withdrawal
bleed and up to 15% have no bleeding.16 This is acceptable
provided the other benefits of HRT, e.g. symptom control,
are realised. Problems may occur with either the timing of
the withdrawal bleed, the length or heaviness of the bleed, or
with breakthrough bleeding. Depending on the age group,
pregnancy should be excluded, compliance checked and
pelvic examination performed unless done recently.
Antibiotics, particularly the broad-spectrum ones, may
interfere with hormone absorption. Synchronisation of the
endogenous ovarian cycle and the exogenous hormone cycle
in perimenopausal women can be difficult."

What's probably going on is that you just aren't getting enough oestrogen (opposed by the progestogen) to make the lining build up enough for a bleed.
