When do I start counting

Hi everyone I have been started on 100 evorel patch and utrogestan for 12 days at 100 vaginally. My dr is happy for me to go on a longer cycle no more than 6 weeks.  When do I start counting, to work out the next cycle?  My period started on the 9th January,  3 days after stopping the utrogestan. Any advice appreciated.

Hi Blueflower - it depends if you are peri or post-meno? If post then you start to count 6 weeks from the date you first started the Utrogestan and then start it again 6 weeks from then and every 6 weeks thereafter. The length of the cycle of course, includes everything - the utrogestan days, the bleeding and then the days on oestrogen only (just like in a normal menstural 28 day cycle when it's all done within the 28 days). If you are peri-menopausal then bleeding could happen at any time and I don't know how it would work, so if  your last period was a natural one without any HRT then if you count that as Day 1 (first day of bleed) you would normally start the utrogestan on Day 15 for 12 days,  for a 28 day cycle. For a 6 weeks cycle ie two weeks longer then just wait another two weeks after Day 15 (ie Day 29) and take it from there. It may not work properly though if you are still very much early to mid peri as your own cycle will take over - HRT does not suppress ovulation. Also you may find that for a longer cycle 100 mg is not sufficient even vaginally. personally I would take the standard cyclical dose of 200 mg even vaginally if I were lengthening the cycle - at least at first - to see what happenes. However if your specialist is happy and is monitoring you then hopefully all will be well and any sign of abnormal bleeding will send you for a scan?

Some women stretch the cycle from 28 days to monthly so take the utrogestan on the frist of every calendar month.

Hello Hurdity Thankyou so much for this information, I am post menopausal, early menopause from age 40 but only on Hrt the last 2 years.  Have tried various regimes but this regime seems to be suiting me at the minute.  I'm still getting to grips with it all.  Thankyou for taking the time to reply I really appreciate it as sometimes it's not explained well by the dr.
