I have to say that I'm surprised the bleeding hasn't settled for you by now. If you can pop back to the specialist for advice and a check up, that would be good - for reassurance if nothing else. As you are only using one pump of gel per day this shouldn't build your womb lining too much. Hope things settle for you soon. DGx

Thought I would check in, in case this helps other women. I went to my referred doctor today who is lovely, after explaining the bleeding, him taking notes from my fibriods and thining womb? He said I needed a hysterectomy. I think I left in mild shock!

If this specialist has reviewed everything carefully and feels a hysterectomy would be your best option then it may be worth seriously considering. The up side of having a hysterectomy would be the ability to have oestrogen only hrt, which means none of the side effects of progesterone - there are downsides e.g slightly higher chance of a prolapse.
Quality of life is important - if a hysterectomy would allow you to get on with your life and then have a bit of oestrogen hrt to control the meno symptoms, then this may be the way to go - after all you have tried every other option with no success.  It's not an easy decision - maybe make an appointment with the nicer GP at your practise to discuss things.
