Painful bleeds
« on: February 04, 2019, 09:57:00 AM »

Hi,

I have been on HRT for about 1 year. I am now 54 and had not gone through the menopause at the time HRT was prescribed - my periods were quite regular and varied between 3 and 8 weeks apart (I would ave around 11 or 12 a year). The main reason for my specialist prescribing HRT was a diagnosis of severe osteopaenia shown on a DEXA scan. She prescribed Evorel 50 patches and 200mg Utrogestan for 12 days a month. I have had some problems absorbing the Utrogestan. If I take it orally I will usually start a bleed about 8 days in. If I take it vaginally I usually manage to take the full 12 days but may have some light spotting from around day 11. I have a good bleed each month - no clotting and not too heavy. The problem I have is that I am experiencing really severe abdominal pains during the bleed, yesterday it even affected the top of my thigh.  Is there an obvious reason for this.

Thank you for your advice x

Hi Sid

Sorry I can't help with this! However if your periods were pretty frequent before you started HRT then your womb lining is being stimulated even more by the extra oestrogen, and the extra progestogen is to ensure that any extra lining will come away so this could be the explanation. I think if your pains continue to be severe  and this persists for several cycles, and much more than before then perhaps it would be wise just to check with the doctor. Have you ever had endometriosis for example? I hope your pains ease soon...

Good advice from Hurdity.

Have you been checked for fibroids recently? These can cause pain.

Hiya

Thank you for your replies. I have never suffered with period problems - never heavy or painful (apart from discomfort during the first couple of hours) and not lasting more than 5 days. Never had endometriosis or had any worries about fibroids either. My current bleeds aren't problematic either, other than the pain which starts about 2 days before the bleed and lasts for the first day or two of the bleed. I suppose I will need to see my GP  :(. She will probably tell me to come off HRT  :'(

Have you had a scan to check for fibroids? Oestrogen can cause them to grow and I didn't know I had mine until a scan for painful and longer bleeds discovered them.
