Off Amitriptyline and on gabapentin - advice please
« on: November 03, 2018, 11:16:45 PM »

Hi I really need advice on my nerve blocking meds

I have nerve pain in my lower back caused by either a disc or facet joint dysfunction.

I refused injections in the spine because Iíve not been x rayed for the diagnosis so opted for Amitriptyline which I have tolerated really well and got my life back.  But due to severe dry eye problems gp phoned me and has replaced Amitriptyline with Gabapentin.

But Iím too scared to take them!! Back pain has now reappeared and I need to make a decision,  my reasons for not starting them are:

Dry mouth side effects - I already suffer from this and canít cope with it getting worse

Dose is 3 times daily - Iím rubbish at regular meds like this, I ideally want a once a day med

Theyíll cause drowsiness- I drive , I work and I have a life, if I take 3 times daily how on earth will I function? At least with Amitriptyline it was once daily and I used them at night as sedative for a good sleep

Thereís more reasons I know but I canít think right now, Iíve never been so nervous about starting something like I am right now

Iím wondering whether to try once daily but then Iíd only be taking 1/3 of the lowest starter dose

Does anyone have good feedback of Gabapentin ? My search on here didnít look as though many got on with it X

Gabapentin has helped me a lot with MS nerve pain. Amitriptyline was useless, just made me brain dead and didn't really help with the pain.

Regarding the three times a day - when I was first prescribed it I was told to taper the dose i.e. 1 day for 3 or 4 days, rising to 2 a day for a week then going onto 3 a day (similar when coming off the pills) Oh what size tablets are you on I was initially on 300mg. After discussion I got 100mg tablets - 500mg to be taken in 3 divided doses. Currently I use 400mg in 2 divided doses which works well as a maintenance dose with no excessive brain fog.

You could just start with one a day and see how you get on.

Thanks Hasty for replying

Have you had any side effects ? I donít normally read the list but searched on here which then prompted me to google !

He said heíd start me on a low dose so I have 100mg 3x day.  I might use my amitriptyline until I donít have work the next day (next Sunday) and try 1x100mg daily then x

I'm sure you'll be fine on the 100mg capsules. I would still go gradually in increasing the dose up to 300mg a day though. The only side effect I have had when I boost the dose (to 500mg a day rather than 400mg) is bigger boobs! They do diminish when back on the lower dose. Strangely enough OH doesn't seem over troubled with this side effect 🙄

Yes I think so but the weight came off after I dropped from the 3 x 300mg a day to up 500 mg a day in 3 divided doses (100+100+ 200 or 300). In reality, assuming no great pain, what I do is 400mg a day in 2 doses (100 +300) When in pain it's back to the 3 doses a day (either 400 or 500mg)

Had another thought re weight gain. When I was taking the higher dose of gabapentin I was a good deal hungrier, so did eat quite a bit more,  which could have something to do with the weight gain. 🙄

At the moment I am on a lower dose and I'm just not as hungry, I struggle to finish a full plate of food.

Must admit I only started using it after I'd retired from going out to work.
You might get some benefit just from using it once a day at night.

Annie+Hasty,
I have degenerative disc disease and the discs at the  bottom of my spine have all fused together so I can no longer have the injections (which sadly didnít help much)
Iím on 75mg amitrip. which isnít working so well any more and Iíve tried (GP wanted me to) another older AD which was useless, so back to amitrip (canít remember its name but wasnít gapapentin). Is gabapentin stronger? My back is only going to deteriorate.
Thanks

Poppi is 75mg too high ? I am sure I read the nerve blocking part comes about on 10 and 20mg doses?

I donít think Gabapentin is stronger, just different side effects I think

I play golf and also have an ankle problem, well a defect really and after golf I can hardly walk, the ankle well and truly seized up and without exaggerating, it feels broken.  I ordered a cheap TENS machine from Lloydís Pharmacy and used it tonight with fantastic results for my ankle, so I will try it on my back when I next play x

Thanks Annie, Iíll stick with amitrip as some days are better than others and just going through a bad patch just now.
Oh I couldnít play golf 🏌️‍♀️! My core building sitting exercise class is my weekly keep fit 😅
I just remembered I have a kind of TENS band thingy from Boots which I will dig out tomorrow. Thanks for jogging my memory!

Hmm Not certain about Gabapentin being stronger than Amitriptyline but I do know that the dose needs to be built up (and lowered) gradually. Got a link here which describes it better

Annie
Why are you not keen to have the injections? I know of 3 people who have them every 6 months and find them very effective. They did nothing for me as they inject between the joints and I had no joints left (a wonderful new spinal consultant X-ray Ďd my back and didnít know where the previous consultant had put the injections 😢)
Hasty, that link is very interesting thanks
