Bleeding on Utrogestan

Hi

I'm nearly 50 and been on various forms of HRT over the past nine years. Last summer I was put on Evorel 75 and utrogestan (orally). I'm very sensitive to progesterone but it wasn't too bad.
Anyway I saw a menopause expert who changed the regime to Evorel 75 with continuous utrogestan 100mg vaginally as she believes im now post menopause.
I've been using this for about four weeks and am bleeding like I never have for years! Very heavy. As I write this I'm sitting on a puppy pad :-( I've had to phone in sick to work.
Is this normal? I was given the impression by the doctor that any bleeding is abnormal?
Any advice would be much appreciated. Thank you.

Thanks Birdie that makes me feel much better! I've hardly bled for years so it's a bit of a shock to the system.

Hi Nik

I'm on my second month of Utrogestan but with Estrogel 2 pumps,  after taking Femoston sequi for the last 7 years.

I had my bleed after the Femoston as usual but have bled every two weeks ever since for at least 7 days and quite heavy.

Can't get an appt with GP until end of this month but in the meantime I have reduced my Estrogel to one pump in the hope my womb lining isn't building up as much - not sure if this will work! 

I suppose it's more difficult to reduce the estrogen when using patches.

Have you contacted your GP?

My GP doesn't have a clue sadly.
I think it's definitely the change in cyclical to continuous utrogestan.
It's making me feel so ill.
I'm seeing the menopause specialist in August but may ring and see if I can get an appointment earlier.
She did suggest long cycle and I suspect that's the next thing to try.

Hi Nik - the most likely reason for your bleed is that the utrogestan you were taking orally was insufficient to protect your womb and so it started to thicken - not enough to cause bleeding, so that now you are taking an effective dose/route of progesterone - this lining is coming away. Eventually this should settle, but if it does not and if you are worried about the excessive bleeding then do please contact your doc and get checked out as there could be other reasons for very heavy bleeding eg fibroids, polyps etc.

Thanks Hurdity. The bleeding has now gone on for four days and it's so heavy I feel totally drained.
I had a scan last month that showed my womb lining to be 3mm and everything else fine.
I'm seeing my GP later really just for peace of mind!

Nik, I've been on a Conti regime for a long time but when my patches went out of production I tried utrogestan with an 50 oestrogen only patch. This precipitated a torrential bleed like I have never experienced in my life...even after three births (and one PPH) and I ended up in A&E because I panicked.
Thank goodness I had found a really excellent meno specialist who was about to fit a Mirena but sent me for a thorough scan first. She was reassuring that this was most likely triggered by the change in hormonal regime and she also said, interestingly, that in her experience (anecdotal obviously but she sees a lot of women like us) in spite of the utrogestan popularity , it doesn't hold the lining as well as other preparations and progestins. My scan showed that any lining which might have built up over the years on my previous regime, had shed and all was normal. The radiologist also told me that the vast majority of bleeds of this nature are due to a hormone change. I was lucky to have been able to be seen privately but if you're in the London area I am happy to recommend. I'm now on Femoston Conti , not bleeding and apart from some cramps early on, and slightly sore nipples I'm doing better. No return of hot flushes etc. My bleeding stopped within about six days of the first heavy bleed so don't lose heart. Personally I didn't feel at all well on utrogestan or I might have given things a bit longer to settle.  Best of luck.

Thank you Rose. That is reassuring. I've felt awful on utrogestan but my menopause specialist says it's the most natural stuff and very few women have problems, therefore, she's keen I persevere. I'm not!
She isn't keen on Mirena either.
I've never had period problems until now - and this is something else. It just seems to come away without warning - very embarrassing.
I'm seeing Louise Newson in Stratford in July and I'm hoping I will get a solution x

I had bleeding on Utrogestan 100mg daily. I had to take 3x Estrogel daily to get a therapeutic level for osteoporosis, and I was bleeding more half of the time. The consultant recommended the Mirena as a next step. It was either that or 200mg Utrogestan. Apart from 5-6 months of bleeding after having the Mirena fitted, I've been OK with the Mirena. So far it's been doing it's job, even now I'm up to 4x Estrogel daily. Apart from the early months the Mirena is OK. I'd rather NOT have a foreign body in my womb BUT it has stopped the bleeding.
