Bleeding when with Utrogestan?
« on: February 09, 2016, 10:11:19 AM »

I take Utrogestan for 25 days at a time. This is month 4. First two months there was no bleeding. Last month I had a period which started on day 11 of taking it. This Mon things feel like they may be heading the same way, plus I am quite irritable. So, clever ladies, why bleed at that point and not after or during the 3 day break? (I have Evorel patches too, 50mg). Thank you

Hi Trufflecat -
Any conti regime will result in some bleeding and spotting in the first 3-6 months and things should settle down in time.

Unfortunately this is the problem I had. I had erratic bleeding and spotting which eventually became a real nuisance - I also got cramps. I believe this is one of the issues with Utro as it isn't as powerful at controlling things as the synthetic progesterones are and the womb lining becomes patchy or thickened and results in sporadic shedding.  I used it orally which I preferred because vaginal use effected my bowels, and my understanding is that oral use does not always deliver a consistent dose and this can give rise to these erratic bleeds.  I am seeing a private specialist gynae and he told me to increase my dose to 200mg daily if the bleeds didn't stop but I did feel rather sedated on this higher dose and still got some bleeding. Using Utro vaginally is supposed to be more efficient, however, using it vaginally in the conti regime is possibly not so good. I did wonder if one could use 100mg Utro orally every other day and vaginally every other day - so alternate days - that this might be better but I wouldn't do this without specialist advice.
Sorry I haven't any real answers - I don't think we are no alone with this problem - I think studies have shown that a percentage of women get erratic and problematic bleeding when using Utro but in your case I would carry on and see what happens over the next 3 months or so unless the bleeds prove to be a real problem - it may well settle.  Dg xxx

Hi DG, thanks for the reply. I will stick with it for now. I will talk to my GP (who knows nothing) next month. Goodness knows why I am on that regime anyway as I am only in peri!

If you are peri meno then your own hormones may be fluctuating a causing this erratic bleeding.  You need to be using a sequi regime.  Dg x

I think that you are right. My GP thinks that this is an OK regime but I think that trying the 7 days or 10 days might be better.

I always bleed earlier than supposed to on Utrogestan. I use it every 2 or 3 months and have my own scant but regular periods in between, might be your 'own' hormones doing their thing?

Trufflecat - I wouldn't do 7 or 10 days unless you are being advised by a gynae.  If you want to try the sequi version then the licensed regime I think is 16 days of oestrogen alone and then add 200mg of Utro for 12 days.  This might control your cycle better - the stronger dose of Utro may work better for you as you are in peri meno.  Dg x

Hi Trufflecat  -  Here is the link which shows the different ways to take Utrogestan - so I'd print this off to show your GP.
