Balancing Oestrogen with Mirena
« on: January 06, 2026, 02:45:51 PM »

Hi everyone!
I'd love any thoughts about this question please...I'm 48 on HRT including a Mirena for the past 3 years (following perimenopausal flooding) and I'm now seemingly going into no periods for the last two months. Maybe this is the beginning of menopause, as when I had a Mirena before I always had a little bit of blood during my period.
My body seems happiest on 3 pumps of Oestrogel a day, as two pumps I get severe brain fog and cognitive slowdown.
My question is: having just gone up 1 pump of gel, do I need supplemental progesterone to balance it out? Does anyone know how much oestrogen the Mirena can be adequate for? I have some Utrogestan tablets so I could take those additionally, but how would I know if my oestrogen levels were tipping the balance? Would I start bleeding again?
Thanks in advance!  :)

There is no known upper limit of estrogen therapy that a mirena cannot oppose. It simply hasn't been investigated because clinically it's a non-issue.

You would have to be really supra-physiological and there is no product that exists, and certainly nothing you would ever get on the NHS that would get you close to this being a concern.

If someone was doing pellets plus mirena they would be sensible to get a scan now and again to check the endometrium but you would run out of skin before you could apply the amount of oestrogel to overpower an IUS.

I have a mirena, the Newson doctor I see is happy to prescribe 200mcg (equivalent to 8 pumps).

I'm on 2mg Sandrena (equivalent to 4 pumps of Oestrogel I think) and NHS menopause specialist wanted me on Mirena before I could increase my oestrogen any further while on 200mg Gerpretix days 15-28. And that was with stopping the oral progesterone. So Mirena should be enough protection.

Intersting info Bombshell, thanks.
