Really sore after switching from cream to Vagifem

I’ve been using oestrogen 0.1% cream for years now internally and externally. It really helped with the itching in particular I was experiencing as I was diagnosed with VA plus vulval dermatitis. I’ve had a flare of of dermatitis despite not changing my routine and thought maybe I had developed a reaction to the oestrogen cream so I spoke to the doctor and asked to go back onto Vagifem which I was on about 5 years ago or more. I just had sex this evening (with usual lubricant Yes OB) and it was so painful I had to stop. Even a finger was uncomfortable, I can’t remember things being this bad ever. Had a look in the mirror and very red and inflamed just inside vagina, was even uncomfortable gently moving things to look. I thought the Vagifem and the cream delivered the same dosage but something has gone wrong in the last 3 weeks and this is the only thing I have changed. I had a swab for thrush last week which was negative. I’m worried I’ve really set myself back or that it could be something else. Has anyone else had this problem when switching over ? I’ve still been using the cream externally as I found the itching got worse when I tried to stop this( I use a very small amount every day externally )
I’m going to try and get an appointment with doctor tomorrow for someone to actually have a look. I’ve also had symptoms of frequent urination at night which is unusual for me. This has only happened previously when I’ve not used the oestrogen cream enough. I’m also on 200mcg patches (I don’t absorb well) and testosterone cream with a Mirena coil. I’m 59. Any advice would be greatly appreciated please

Get that oestrogen 0.1% up there now, again at lunch time and this evening, ring the Surgery to request more.  Sometimes when the vaginal walls are dry anything inserted might sting.  So softly softly approach.

A pessary may be too rigid at the moment.

Maybe the area just inside the vagina is not being reached with the vagifem. Three weeks is not long for vagifem to work fully.

Could you try using the cream externally still and just up inside the vagina to cover where you are finding it sore for now then see if you need to do that later on.

When i had a hissy fit and stopped using Estriol itching came back with a vengance worse than when i started out.
