Confused about progesterone dose

Hi everyone,

I’m hoping someone might share their experience or thoughts on this. I’ve recently been put back on Bijuve (1mg estradiol + 100mg progesterone, daily) after struggling emotionally with 14 days/month of 200mg Utrogestan. I’m now also using one pump of Estrogel every morning, because I feel so much better on a bit more oestrogen.

So my daily hormone intake is roughly:

1.75–2mg of estradiol (from Bijuve + Estrogel)
100mg of daily micronised progesterone (from Bijuve)

I’ve read some sources saying the ratio of oestrogen to progesterone doesn’t matter — that 100mg of progesterone daily is sufficient for endometrial protection, regardless of a slightly higher oestrogen dose. But I’ve also seen others suggest that adding extra oestrogen should mean adjusting the progesterone too.

Has anyone else been on this kind of combo?

Hi if the progesterone in Bijuve is the same dosage as Utrogestan, then 100mg can be used up to 3 pumps of Oestrogel or a 75 patch or 3mg oral oestrogen.

Any high doses of oestrogen need a dosage of 200mg of Utrogestan / micronised progesterone.

This is guidance from the BMS.

We are all different and it is difficult to say a particular dose of one formulation equates to a particular dose of another across the board due to individual variation in absorption and metabolism.

Additionally the amount of progesterone that controls one person's endometrium at a given level of estrogen may be insufficient for another, so generally manufacturers and prescribers tend to err on the side of caution and have everyone on a dose that protects the highest risk women, even though this means others are over treated.

You are still on a low dose and in all probability that dose of progesterone should cover you.

However my advice to anyone on a more individualised regimen, particularly when mixing and matching different formulations or adding extra estrogen to a fixed dose combined product would be to get a scan once a year or so to make sure you aren't developing hyperplasia.

Hi pages 13 and 15 give you the doses and how much progesterone is advised.

4 pumps of gel or 100 patch is regarded as a high dose.
