I'm on evorel sequi.

I changed from combined patch to estradiol only patch two days ago. 

Yesterday I visited a relative and while making tea in the kitchen, and her wittering on, I suddenly felt like I couldn't cope, with her talking incessantly (I'm used to it, it was something in me), then my head kept going dizzy every time I moved.  I made the tea, sat down and felt like there was a tremor all through my body.  My ears were ringing as well. 

I've had this before but I'm trying to work out what it is, I'm going to keep a diary.

As I'd just switched back to estradiol, could it be that oestrogen levels are very low just now.  Or could it be the estradiol itself?


I was also very dehydrated, my own fault.  Still was a bit today (Sat) and while it's all lessened, keep getting these surges in my tummy and chest and when that happens, simultaneously a bit of vertigo in the head and a feeling of palpitations (pounding).  Heart tests 5 years ago were normal.  This happens at certain points in the cycle so I'm wondering for now what possible menopause symptom it might be?  I don't get hot at all, never have.

Period usually arrives on first estradiol patch but it's not here yet..

I'm 54, feel like my cycle is still my own though last month was the first time I'd say my period was 'different,' less, and darker in colour (not to be tmi here).  If any of this helps with working it out.

I do actually have a blood test scheduled anyway.

I know I don't reply to everything but I appreciate everyone, I have a lot of fatigue / brain fog most of the time and I do read replies and will answer when I can.  Just (hopefully) going through a bad time with hormones.

period has arrived today and head is a lot better, but the symptoms are severe, not mild, so need to solve this as worry there could be long term effects down the line.

Vasomotor symptoms related to hypoestrogenism.

Not everyone gets hot flushes but what you are describing comes under the umbrella of vasomotor instability.

A key giveaway is worsening just prior to menstruation when estrogen is lowest and/or falling rapidly.

The only perimenopause symptom I ever had was premenstrual vertigo, whilst I had lost access to my combined pill during the pandemic.

For me this was completely resolved by getting back on the pill which keeps me bleed free and hormonally stable with a healthy level of non-fluctuating estrogen.

Aiming for stable, adequate estradiol levels is the best way to manage this.

It sounds to me like progesterone withdrawal though strange if you don't normally experience this. Because you felt this sensation 2 days after changing from a combi patch - the timing fits this explanation. Progesterone withdrawal affects women in different ways and is one of the components of the pms we experienced during our fertile years when menstruating normally. For me prog withdrawal affects me differently in some cycles. Also progestogen affects blood sugar - can't remember the mehcanism but involves insulin I think? So during the progestogen phase and on withdrawal there are phsyiological changes in the body which can give rise to symptoms as the body adjusts.

The HRT cycle that you are on won't affect oestrogen levels - it is only during your natural hormonal cycle, and ovulation that estradiol levels change. Evorel conti combi patches deliver the same dose of oestrogen all the time. However if you ovulated then your own hormones will come into play as the patches do not suppress the cycle, and these changes can override the effects of HRT and also cause unusual symptoms.

I do sometimes get some transitory dizziness and vertigo during or just after the progesterone phase.

Whereabouts in menopause are you - sorry I can't remember? How frequent were your periods before starting HRT and how long have you been taking it? Will give us a clue about whether your own cycle may be coming into play perhaps?

I get this sort of stuff a LOT. If you've ever seen any of my threads, you'll know I'm in hell.

I would say that mine are often at their worst both pre period and early follicular. So when E has dropped and is low / climbing again. Dizziness and vertigo with ear pressure are high for me at this time

You say you never get hot. I dont display the typical symptoms of low E either. But I am highly symptomatic most of the month, but especially so when around my period. I dont believe, for me, this is low E. I think my body is hypersensitive to changes.

The fact you've just made a change - would suggest this is your issue here.
