Unexplained bleeding

Hi just wanted some advice please as im new to the group. Im 52 yrs old and have been on the evorel patches and progesterone tablets for about 2 yrs. With no problems until recently. I was having regular periods prior to goung on hrt. For a couple of weeks i have experienced spotting so i went to the dr who said to come off hrt straight away and see what happens. If no better go back in 6 weeks and he will send me for tests. The next day after stopping the hrt i experinced heavy bleeding and i was in excrutiating pain. That lasted for about 3 days and now im back to spotting again. I also have a fibroid so not sure what is going on. I would appretiate any advice as its such a worry.  Many thanks.

That's nonsense, you do not have to stop HRT for a bit of spotting.

Depending on how long it is since your last bleed the appropriate management would be;

1/. do nothing/observation only if you are still in perimenopause (<12m since last bleed) as irregular bleeding is common in this stage

2/.increase progestogen in the first instance in a low risk postmenopausal woman (>12m since last bleed).

3/. if any concerns or persistent bleeding, refer for scan +/- hysteroscopy.

None of these require you to come off HRT.

Up to date guidelines are available for clinicians on this however not all are aware and we often have to advocate for ourselves.

Other causes of spotting/bleeding should also be excluded eg vaginal atrophy, check cervical screening up to date.

Im not sure what stage im at as i had periods before i went on hrt and regular bleeds while on hrt up until recently. I will consult my surgery to see if there is a dr that specialises in this and not just a regular dr. It is so difficult to even see a dr face to face at the moment.

I was late menopause and had a lot of breakthrough bleeding whilst on HHRT. I had numerous scans due to the bleeding and the drs presuming I was post menopausal (I am now) At no point did anyone suggest I stop taking HRT while they did investigations.

I just wish drs were all trained on the menopause and how to help women the correct way. I just dont know which way to turn at the moment it is so frustrating.
