Slight bleeds

Help. I am 58, four years post menopausal. For three years a 50 patch estrogen and 100 utrogestan oral worked. Then I started to bleed very lightly every now and then. Had two ultrasounds and two smears, everything ok but gynie preferred changing my meds.
So for ten months I have been on four sprays lenzetto and 100 utrogestan but my flashes came back and very tearful.
So a few days ago I had enough. Stopped the lenzetto and put on a 50 patch. Flushes better but lo and behold after only two days I am staining blood.
200 utrogestan bloats and depresses me. That's not an option.
Any advise please or why does the patch cause me to bleed so quickly 😔

Bleeding can happen every time you change hrt . Four sprays of Lenzetto is more oestrogen than a 50 patch but it depends how you absorb it.

Suggestions would be to change the progesterone to Provera or have a Mirena coil.

If your symptoms were well-managed before there was no reason to change your HRT estrogen.

Have you investigated a Mirena coil? It replaces utrogestan. Most women love them. Totally stops all bleeding, and lasts 5 years. Set it and forget it...

I was taken off the patches due to slight bleeding but I felt better on patches. Four sprays of Lenzetto made my flushes return and low mood.

You could try the progesterone vaginally instead - this avoids first pass metabolism and delivers a higher dose to the endometrium.

Alternatively, and this might cost you more but could be worth trying, is changing your patch more often eg every 2 days.

The manufacturers claim they last 3-4 days but with real world use they are often flagging after less than this especially if crinkled or detaching around the edges ( I used them for IVF)

Also, if the bleeding has been fully investigated and is benign, it doesn't necessarily need to be eradicated at the expense of your quality of life. Feeling well and wearing a panty liner is a legitimate option.
