Utrogestan to sync or not?
« on: July 29, 2024, 02:29:30 PM »

Last month all I had was 4 days of spotting 5 days after stopping the utrogestan. Now I've got a proper period just 7 days after starting the utrogestan. Do I stop the utrogestan a week early and restart it on day 15 or do I just continue and finish the 14 day course I'm currently on? My big worry is will I now have a withdrawal bleed in about a week's time in addition to my current period. It seems like an impossible task in perimenopause to attempt to sync the withdrawal bleed with your own cycle  :(

Hi K45

Yes it is an impossible task to sync withdrawal bleed with your own cycle because the very definition medically of peri-menopause is irregular cycles. In the early peri-menopause when your cycle is still quite strong there may be cycles where you bleed twice but gradually this should settle as ovulation becomes less frequent and the bleed becomes more usually, the HRT withdrawal bleed towards the end or shortly after stopping the utrogestan.

The advice generally is always to continue with the utrogestan cycle for the full course to ensure your endometrium is fully protected as you won't know which months you ovulate and therefore producing your own progesterone.

If after a few months the bleed is consistently occurring at the same "wrong" time in your cycle then yes it's possible to tweak eg by a week to try to sync - but as I said your own cycle may well break through if ovulaton is irregular.

What dose of oestrogen and progesterone are you taking and is the prog 12 days per 28 day cycle?

Hi Hurdity,

Many thanks for your helpful response. It's all rather confusing this whole perimenopause malarkey! I think I'll like you say just keep taking the utrogestan for the complete course and review in a few months.  GP prescribed 2 pumps oestrogel daily and 200mg utrogestan 14 days per 28 day cycle. Like most in perimenopause my cycle varies enormously so even my period app is getting baffled at this point! The first few months everything seemed to be going ok with it being a withdrawal bleed at the expected time so I'm hoping at some point things will settle down again.

Thanks again  for the reassurance  :)
Hope you're enjoying this glorious weather, had several fans surrounding me at work lol x
