spotting most days
« on: November 30, 2023, 07:06:42 PM »

Hi all,
Wondering if anyone here has had similar?
Getting a bit fed up with having spotting most days for the last five months.
I'm post menopausal (58) and been on oestrogel/uterogestan for 5 years (4 pumps/one tablet per day), mostly without any problems.
Last year I have a case of post menopausal bleeding, had TV scan of uterus and ovaries which confirmed everything OK. Consultant of opinion that I'm at low risk of developing endometrial cancer.
My GP recommended that I up the dose of utrogestan to two per day. Made me feel tired and low, but persisted with it for several months.
Got fed up with feeling so low/tired that I tried taking one tablet vaginally. That was fine but, when I switched back to taking two orally (in preparation for smear test), I experienced v heavy bleeding. This stopped, but since then have experienced spotting most days.
Newson recently suggested reducing dose ot one tablet per day, and considering mirena coil. Now taking one per day - mood/energy much improved but still spotting (urgh).
Keen on getting mirena but have experienced cervical shock in past am bit worried about going into shock again.
I'm seeing my GP on Tuesday and will probably be sent for scans/hysteroscopy etc due to bleeding.
Anyone had similar? Is Mirena likely to help?
Thanks.

Hi!  I suspect that you aren't producing enough hormones to stop the bleed.

MayB put 'mirena coil' into the search box to read threads that pop up: make notes.

Also put your HRT regime: individually: into the search box.

:welcomemm:  I'm sure that someone will be along with advice.

A mirena is usually the best option to control problem bleeding. Are you taking utro vaginally again? It's possible the scan will show a cause for it eg fibroid.

Not worse.  Health is good.  And feels a lot better since I reduced the utrogestan to one per day.  Just annoyed with the spotting.  Thanks for asking BTW

I recently saw a doctor say on a podcast that the local progestrogenic effect is up to 10x more when micronised progesterone is taken vaginally compared to orally, so just doubling your dose to go back to taking it orally probably resulted in a significant drop in dose at the uterine level, hence the spotting.
Most doctors seem to allow a halving of the dose if you take it vaginally, it tends to still be better at stopping spotting than oral and the side effects are reduced by a lot.
Have you tried going back to the vaginal route of admin?

Ah, that's helpful.  Thanks for your response. I'm tempted but a bit wary of going back to vaginal route of admin as, if I need to change back due to oral, based on past experience, it will result in another blood bath...
Seeing my GP tomorrow so I'll see what she says.
Must say though, absolutely no spotting with the vaginal route, just wish it wasn't so messy.
