Estradiol doses for gel vs patches
« on: October 18, 2023, 10:42:49 AM »

Ladies, a quick question. I've been moved from gel to patches but a bit confused on the estrogen dose.
I was on 1 pump/day Oestrogel = 750 micrograms estradiol per pump.
Now on Evorel 25 patches = 25 micrograms estradiol/24 hrs

That looks like a massive difference, what am I missing? Does the transdermal gel not all get absorbed? Or what would be the reason these amounts are so hugely different??

Hi that is right 1 pump of Oestrogel is roughly equivalent to a 25 patch.

There’s a table in this document on page 2 that shows you equivalent doses.

It's all very random really, because you might absorb one product much better than the other. So whilst they can tell us about equivalents if you're not absorbing something well but are another product, then it results in a big change - regardless.... I think the approach really (for me) has become 'throw everything at it and see what sticks'. Highly scientific.

Thanks Dotty - really useful! I'll bookmark that one!

Haha joziel, I think that's where I'm at, just starting this HRT journey and can see there will be a LOT of that!! Currently trying patches rather than gel to avoid the migraines that have started. Also the absolute raging libido (sorry if TMI!!!) seems to have eased off, but I am now feeling nauseous..... as soon as one thing improves, another one goes off the rails. Oh well! At least I am sleeping nights as the hot flushes have thankfully stopped. :D

That looks like a massive difference, what am I missing? Does the transdermal gel not all get absorbed? Or what would be the reason these amounts are so hugely different??

Also to add a technical point to what has already been posted about how estradiol is absorbed differently and to answer your question, the gel figure of 750 mcg is the TOTAL amount of estradiol in that quantity of gel, whereas the patch dose is given as the AVERAGE amount of estradiol absorbed into the body over a 24 hour period, which is different from the total amount in the patch. The total amount of estradiol in a 25 mcg Evorel patch (according to the product info) is 1600 mcg.

Hope it works for you
