Fsh tell if post menopause
« on: May 20, 2023, 06:42:53 AM »

Am 53 years old. Had bloods don't last May before started hrt and oestrodial levels were 275., Fsh was 70 and LH was 37. I had what seemed like periods until January 2023( I hadn't bled in years as was on cerazette which stopped at same time started hrt). I asked to go on continuous hrt as wanted  constant progesterome. Saw a gynae last week and she said that based on my fsh/LH last May, I was post meno at that stage and shouldn't be spotting infrequently now( reason I went to her). I argued that because my natural oestrodial levels were 275 back then, I must have been peri? She said you go by Fsh and LH levels and because mine were so high I was post meno a year ago. If I was peri back then, then it's an explanation for the spotting surely and doesn't warrant further investigation? She wants me to have a hysteroscopy but under general anaesthetic . I want to avoid if unnecessary and see if spotting stops in the next year or so..I guess just curious if the fsh and LH can be used as she says

I think GPs tend to go by the FSH - or they did when I was told I was post meno quite a lot of years ago.

My FSH was well over 100 and I'd missed three periods age 51. My GP said that I would never have another period but I then went on to have fairly regular ones for over a year. I've never known what my LH was.

Very interesting. So can spike even during peri. I started on continuous hrt last May and had what seemed like regular albeit light periods until December. I wonder if that could just have breakthrough bleeding. It was pretty much every 30 days and lasted for 5 or so days so surely was a period.

I don't think it's just as accurate as they make out, more of an indication than definitive. As you've been over 6 months on conti could you ask for a scan first? It's non invasive and if everything is normal you can then decide if you want a hysteroscopy.

When you had your fhs level tested how long had you been without periods?

Was taking cerazette at the time so no periods for years. Went straight onto continuous ( even though I believed at time I was  late peri,I just wanted the utrogestan nightly for sleep). Could the monthly bleeds I had subsequently have been periods or just caused by the hrt? They were so regular for 6 months though, it's hard not to believe they were actual periods

Had scan( internal and external) and showed fibroids, lining ok etc. Gynae still wants me to do hysteroscopy as says I'm post menopausal so shouldn't be bleeding and need to rule out cancer(I guess?) It's private so maybe just wants the money! I'd prefer to wait another 6 months. The spotting is getting less and less as the months pass and I haven't had anything even resembling a period since December

Oh, how I recognise this scenario. Aged 60 asked gp if I should be concerned that the hrt cycle only induced a bleed once in 2or 3 hrt cycles and mentioned the 2 recent periods I'd had. Urgent referral etc etc, the 'not bleeding I should have done' had turned into 'can't possibly be having periods as over 54 therefore it must be cancer'. Surprisingly enough everything was normal. They said if I'd been 10 years younger they wouldn't have done anything. If you decide to have a hysteroscopy I'd suggest taking a pain killer first as it hurts. Do you mean you have fibroids or you'reclear of them as they can be a reason for bleeding? Imo if you want peace of mind then go for it but it may be your doctor erring on the side of caution.

Thanks Sheila, I definitely have a number of fibroids, the biggest is 4cm I think. Yeah I don't know why gynae didn't mention those. She's insisting I have general anaesthetic for it but no way on earth am I going through that. I may take a valium ( if GP prescribes for me ) and painkiller if do decide to go ahead. I'm waiting it out for a few months first before. I've had spotting for 3 years ( also while was on cerazette) so if it is sinister, then it's pretty advanced!
