HRT or no HRT????
« on: December 05, 2022, 04:35:01 PM »

Hi I am 51 and was told earlier this year that I am post menopause. I was taking Cerazette for 12 years and had no periods so don't know if my cycle had changed. I went for the blood tests as I was having other symptoms - Joint pain, palpitations, some flushes and terrible anxiety.

I was prescribed Utrogestan for 25 days and Estradiol 2 pumps a day. I did this for 3 months and started with really heavy bleeding for 14 days+ so went back to GP who said to take Utrogestan all the time and this would stop the bleeding. I tried this for a couple of months but it did not stop the bleeding and last month it was so bad I could not go into work and would leak through my clothes, this really got me down so I stopped taking everything as I felt so terrible!!

The problem is all the other symptoms have come back and much worse the anxiety and palpitations are so bad. Crying on public transport one particular high point of this month :(

I just wanted to ask if the bleeding does stop? and if so how many months did people bleed for? Is it worth trying just 1 pump of gel and the Utrogestan in hope that it helps with the symptoms but I don't bleed as much?

I don't feel like my GP can really help and I am not sure what to do now?

Hi JuCoo2308,

I just wanted to say hello.

I can't help with the bleeding, however to help your symptoms, I can't see any reason why you shouldn't start on one pump of gel; they should help with the flushes etc.

Some other more experienced ladies will be along to help xx

Hi JuCoo. Just a quick reply but firstly because you have been on the BCP you have no real idea if you are post menopausal (one year without a natural period) and I think you're GP was wrong to decide this. At the age of 51 you could very well still have your own cycle going on. I was still having regular periods at that age. Someone else will be along soon with more of an idea of what might help but you are not alone with this problem. We'll try to get you back on track so you will feel better again

As Taz says, you don’t know where you are in menopause so you’d be better taking 200mg of Utrogestan for 12 days of every month. Your GP is incorrect to say that taking it every day will stop your periods . HRT doesn’t work like that x

If you want to be bleed free the best option is a mirena which usually stops it even if you're peri. Some women aren't meno til their early 60s so at your age you could well be peri. I agree with the advice to try a sequi regime.

Hi JuCoo2308

Welcome to MM. I am 66 now and stopped using systemic HRT at the age of sixty because, after 25 years of HRT due to premature menopause, sadly my body simply wasn't tolerating HRT anymore. I am still getting hot flushes and many of the usual meno symptoms and I don't expect this to subside.I used Oestrogel successfully on a sequential basis(having progesterone tablets for 10 days each month and then having a withdrawal bleed after that) and this suited me for around 10 years and enabled me to function well. I then tried continuous HRT ( a no bleed regime) but didn't' get on this at all well. In the end I had a Mirena fitted and used Oestrogel daily and this was probably the best HRT combo I ever had.

Over the years I'd tried every conceivable HRT type going and in my late 50s I tried Utrogestan with Oestrogen but, like you, sadly I experienced bleeding issues. I felt great on the Utrogestan but I believe it isn't as strong as the synthetic progesterones and therefore  doesn't prevent the uterine lining building up so resulting in bleeding. Some women use Utrogestan vaginally on a sequential basis very successfully but I found this aggravated my urogenital atrophy.

I agree with sheila99 and the others, you could still be peri menopausal, and if you've had children, then the Mirena could be a very good option for you - giving a bleed free regime with minimal side effects. Alternatively, Utrogestan at 200mg for 12 days each month might work better for you but you will get a bleed each month. I would also suggest you start with just one pump of gel each day and see if that is sufficient to control meno symptoms and only increase if needed. Good luck  DG xxx
