Advice re Utrogestan please?
« on: July 03, 2019, 10:01:03 AM »

Hello again ladies

I have just started systemic HRT this week after years of trying to deal with worsening symptoms naturally- fighting a losing battle!

I am 56 and 7 years post menopause. Have been prescribed Estrogel 2 pumps daily which I am to take for a month to see how/if my symptoms improve, and then add Utrogestan 100mg daily.

For last three years I have been using Estriol cream vaginally for atrophy ( following years of repeated UTI's for which my doctor dished out anti bios. It took my own research on here to realise it was atrophy that was causing it !)

My question is this, I would like to take the progestogen vaginally as I would like to minimise potential side effects. Obviously as I am using the estriol cream twice a week also vaginally, is it ok to use both at same time? Or, should I hope that the systemic oestrogen will kick in and I won't need the cream anymore.

Am sure there is a sensible answer to this but i'm really so confused with this new regime!

Any advice/opinions gratefully received.

Many thanks in advance

Hi westie,
I am 57 and use both of those but on a different system.  I use 4 pumps and the utro on a cycle 12 days vaginally.  I have to keep progesterone to a minimum and cannot take it continuously.  I don't need the local cream I never have but I know you can use them together.  There are plenty of ladies on the forum who do, there is no issue.  One of them maybe along soon to explain how they do it, but I just think you crack on an apply as usual and push your utro as far up as it will go.

Hi you could start by taking it orally and see how you go.  I take it orally every night and I have no problems or side effects from it . x

Thanks for your replies ladies.

Dotty, if you take the utro for 12 days do you have to double the dose.

Hi Westie,
Initial I took it orally but it gave me terrible nausea, just like morning sickness, I couldn't eat until lunchtime. I then started to use it vaginally and all that stopped in a couple of days. Using it vaginally for me has made my bladder a little more sensitive towards the end of the cycle, but not always. It definitely works better vaginally for me.

Hi I know the question wasn't for me 😄 but yes, if you take it for 12 days then the dose should be double . Although I think some ladies still only take 100mg for 12 days. Xx

As dotty says - 12 days should be 200mg but if you take it vaginally some ladies use 100mg for 12 days because it doesn't get "diluted" by stomach acid - or so the theory goes so 100mg vaginally is thought to be more potent than 200mg orally.
That's as I understand it.

Thanks so much for the replies. I'll try 100 Megs vaginally to start off with and see how I go.

Still not sure whether it's ok to use alongside estriol ?

When I used to use the discontinued Orthgynest pessaries, when I was on the utrogestan part of my (long) cycle I swapped to Vagifem for this stage as it is a dry tablet and sticks to the vaginal walls so in my head it was still stuck there giving out estradiol all the while I was using the progesterone capsules ( vaginally). I would insert the utro at night and then approx 12 hours ( ish!) later I would insert a Vagifem on the days I needed it. Npew I do this all the time with Vagifem as my main VA tool. Once my period starts now I swap to estriol cream ( not ovestin, but the generic 0.01%) and smear it on the tampons every time I change - as otherwise I get very dry... I don't think there are any studies on this (ie mixing them together!) as using Utrogestan vaginally for HRT is off licence in UK. I'm sure lots of private gynaes endorse it though.
