HRT Mirena & cestrogel
« on: January 09, 2018, 10:09:03 AM »

Postmenopausal commenced HRT in November. Had old Mirena removed. Was used for contraception. Warned post menopause but could get pregnant as not had full 12 months without period.
Initially was going for Mirena but reinsertion problematic in November. Therefore procedure aborted and commenced on
Femseven conti patch.
Unfortunately I had a convulsive seizure on the 1st day of treatment (nothing suggests HRT caused the seizure). Hot flushes and sleeplessness improved quite quickly within days on the HRT.
However I have felt quite flat in mood initially I thought it was depression following seizure but has the weeks went by I was not improving and then realised it could be related to HRT.
Today I have had the Mirena fitted and Have had the cestrogel prescribed. I requested the gel as I think it will give me greater control of symptoms.
Do I remove the current conti patch ?
How quickly will the Mirena release progesterone ?
What have forum members thought about the Mirena & estrogel combination of treatments for postmenopause ?

Love my Estrogel and Mirena combo!
Had Mirena fitted one year post meno and started estradot patches straight away, didn't get on with them and changed to the Estrogel.
I would move the patch straight away.

I had Mirena fitted one year post meno too, and have only used oestrogel, so have nothing to compare it to.
I am considering upping my oestrogel dose, as still having vaginal issues, and hair, skin and nail dryness.
I've always just used two pumps, but am wondering if three or four pumps might improve things further?

I do have occasional spotting from Mirena, but only very light, not enough to have to use anything, and only for a few days.

Hi and welcome to MM TAW
Yes, remove the combo patch and then start the Oestrogel. I'd start with just one pump per day for a week or two and then increase to 2 pumps per day.  You may find you get some bleeding or spotting at first and you may feel a bit sedated as well but this should all settle over the next few weeks.  The Mirena with Oestrogel combo can be one of the easiest HRT regimes to handle - fewer side effects and least hassle. I had this combo for 4 years in my 50s and it was the best HRT regime I ever used. DG x

I'm postmenopausal too and use mirena, estrogel and testim, this combo is great for me as I didn't get on with any other progesterone.

Thank you for the positive replies started after removing patch yesterday. Have started on two pumps prior to bed.
Slight achy legs but otherwise ok. Comfortable with Mirena today.

Hi Taw, I'm considering the Mirena as I don't do well on any of the progesterones. I tried femseven and felt really flat too. Can you update as you go along as I'm nervous about low mood but thinking the oestrogen will counteract that. My menopause specialist has said to use 75mcg everol though so hoping my Gp will prescribe oestrogel if I ask as it appears to absorb better with most on here. Think I'm going to have the Mirena so fingers crossed!! X

Update
continued on the
Estrogel 2 pumps daily before bed &  Mirena.
Mood has lifted.
Noticeable aches in thighs lower back pelvis region thought it was because I've gained lots of weight, reduced exercise.
Any ideas how to relieve or possible cause.

The ache in lower back and thighs could well be a lower spine problem and possibly a compressed disk. Try to get back to doing some more exercise. Brisk walking really helps my lower spine issues. If you can get to Pilates or yoga classes that may also help.
Extra weight won't help so try to eat smaller portions. As the HRT works it's magic, you should acquire more energy for exercise but the rheumatologist I saw about my back told me “you must keep moving and don't sit for too long”. DG x

TAW - I wouldn't do any running until you are sure your back is fully better - running tends to jar the vertebrae so can make things worse. DG x
