Hi Everyone

I'm new to the forum and have been through lots of the information on here, which is very helpful.

However, today, I'm feeling extremely nauseous and miserable, and I'm looking for some advice / support.

I'm 56 and had my last period in the month of my 53rd birthday. For the last couple of years, I've been using bio-identical HRT prescribed by a specialist clinic but recent blood tests taken howed that I wasn't absorbing the cream. I tried but didn't get on with their lozenges. The doctor at the clinic (GP) suggested that I switched to Estraderm MX25 patches and Utrogestan 100mg daily. I started using these but at the same time was being put on fluoxetine for depression and anxiety and suffered dreadful nausea and dizziness. My doctor advised me to stop using the new HRT (after 4 days) and go back to the combined cream whilst my body got used to the fluoxetine. The side effects from the fluoxetine (nausea) were so bad that I had to start on 4mg a day (using liquid medicine usually used for children) and gradually increase this to 20mg (the usual starting dose) over several weeks.

I am now established on the fluoxetine, which is helping with my depression and anxiety, so I decided to see a private gynae at a well-known London hormone clinic. He prescribed Estradot 50 mcg patches, Utrogestan 100mg daily and Andofeme 1% testosterone cream (0.5ml daily) Four days after starting on this regime, I have terrible nausea, vomited last night, loose bowel movements and extreme tiredness. I emailed the clinic this morning and one of the doctors left me a message to say that it's unlikely that the HRT is causing these side effects and that I should persist for another week.

From what I've read on here, it would appear that these side effects are quite common with Utrogestan? The patient information leaflet says that the side effects should pass once the body adjusts to the drug. I'd be interested to hear whether anyone has found this to be the case and, if so, how long it normally takes. If there's one thing I can't tolerate, it's nausea! I think I'd rather go back to the hot flushes that have to put up with this!

The gynae has suggested I persevere for another week and if I'm no better then go in for a face-to-face consultation.

My hormone blood levels before starting this new regime (despite being on combined bio-identical HRT for 2 years) were:

17-beta Oestradiol - 59 pmol/L
Progesterone - 4.6 nmol/L
Testosterone/shbg - 1.7

Any advice or support gratefully received  :'(

Oh poor you.  Hopefully someone with HRT knowledge will be along soon!

Thanks CLKD. I'd love to hear from anyone who has experience of the side effects of Utrogestan going away after a while (hopefully not too long!) I was warned about the side effects of fluoxetine and encouraged to persevere. That persistance paid off and I don't have any side effects with that medication now. I'm just hoping the same will happen with the HRT.....

Oh dear (for many reasons!)!

Firstly - no wonder you haven't been feeling right - none of this treatment apart from the last one - is really ideal for menopause.

The effects of the BHRT that you were prescribed  - are typical and this is not recommended by the mainstream medical profession. It is extremely expensive, non-standardised - so that you don't know how much of each hormone you are getting, and it is not regulated. Creams are not well absorbed but yet these clinics continue to get away with charging exhorbitant prices to women and prescribe them!!!

Anyway at least you realised that - but unfortunately the doc at the clinic prescribed far too low a dose of oestrogen so no wonder you were still feeling down.

Please don't take this the wrong way but if your depression was new onset - and not due to life events ( or even if it was) you should have been given a proper trial of HRT at a suitable dose BEFORE drugs like Fluoxetine were consdiered and given (unless there was a compelling overriding reason in your case to do so). The NICE Guidelines on Menopause are very clear about this.

Your doctor should have put you on at least a 50 mcg patch and not prescribed the AD (subject to your situation).

The other thing is you are quite right - one of the most common side effects of utrogestan is nausea from oral use and in continental Europe it is licensed for vaginal use for women who experience this side effect - and many doctors and gynaecologists here in UK also prescribe it for vaginal use when nausea and fatigue etc are experienced severely. In addition you are taking it every day - and although this no-bleed regime is usually recommended for women who are post-menopause, in practice many women who take utrogestan (and especially those who get the side effects) take it cyclically to avoid the continuous side effects like you are experiencing - quite extremely. I presume you were taking it last thing at night and on an empty stomach?

You oestrogen levels are still low but hopefully they will build up once you have been using the Estradot patch for a few weeks. You may well need to increase the dose a bit more depending on how you feel? In your position and if you can bear to have a bleed, I would try the utrogestan vaginally and take it for 11 days per month. The licensed dose is 200 mg but if you are seeing a specialist and your bleeds are not too  heavy then you might be able to reduce this. You should not get vomiting when taking vaginally unless the progesterone itself is causing this? If you don't want to have a bleed then I'm not sure where to go from here, if like most women you don't want to take it vaginally on a continuous basis?

The other alternative would be the Mirena coil which delivers most of the progestogen locally and then you can add in whatever dose of oestrogen you like.

I am interested in the Androfeme as this is the only product available for women - but not licensed yet in UK. Creams are not especially well absorbed but with testosterone it is not so crucial to reach certain levels - well I should say that insufficient is known about this!

Ideally once you have got yourself sorted on the right regime you might be able to come off the Fluoxetine - because it is most likely that the hormones are causing or exacerbating your depression and anxiety if it is new onset.

It's an interesting point you make about ADs and the menopause. My depression and anxiety came on about 9 months ago, whilst I was taking the BHRT and there were events that precipitated this. I don't have a good relationship with my GP - mainly because it's almost impossible to get an appointment and when I do, I feel as though my time's up before I've had chance to explain my concerns - so I have been seeing different consultants privately, hence the lack of joined up care. The ADs and a benzodiazepine were prescribed by a psychiatrist after I was referred by a clinical psychologist, so quite separate from the HRT clinic.

I feel a lot more confident now that I'm seeing a proper menopause gynae and I will, perhaps, suggest that he liaises with my psychiatrist over meds.

I wasn't aware before reading this forum that the Utrogestan doesn't have to be taken continuously or that I could still have withdrawal bleeds at my age. The vaginal option is also worth considering. I will definitely discuss all of this with the gynae when i see him next.

Thank you for your advice and reassurance. I feel better equipped to go into the next consultation with my gynae and hopefully, I won't have to cope with this deadful nausea for long!

Jo, sorry to tag onto this thread so late but your oestrogen level is very low and could explain why you are feeling so bad.  My immediate thoughts are too much progesterone and not enough oestrogen.  You might want to consider swapping patches for Oestrogel too because it give you more control over the dose.  It's very flexible and you can quickly increase or reduce the dose as required. 

I always take Utrogestan vaginally too - I think it is only the UK that doesn't licence it to be used vaginally.

Thanks Mary. I will bear that in mind when I go for my next consultation. Taking Utrogestan vaginally sounds like a good option.

I never knew before this happened to me that the menopause could have such an effect on mood. I hadn't really thought past hot flushes and they were the least of my concerns!
