Many thanks for your reply which was well informed and thorough.  Much appreciated as always.

I have just returned from seeing the specialist and will update the forum when I've had a rest and a cup of tea!
The appointment was a little disappointing actually but perhaps I'm expecting ( and certainly hoping ) for too much.
