Hi all

Am new to the forums, but already is a god send to hear everyones experiences.   I am 41 and was officially diagnosed as being peri menapausal 3 weeks ago via blood tests.  I have had the mirena coil for about 18 months, which after a settling down period of 6 months has helped immensely with the severe periods I was getting.  Started getting awful hot flashes and night sweats about 6 months ago (about 10-15 a day).   Started taking St Johns Wort and Black Cohosh for a few months which alleviated the flashes and distrubed sleep completely, but then they came back with avegence.  Have been taking Elleste Solo 1mg for two weeks now after going to the docs to chat about results of blood tests.  The hot flashes are starting to ease, but I feel aweful, extremely fatigued and have severe muscle pain, heart palputations, some dizziness and very low mood to the point where I took yesterday and today off of work and am trying to get a doctors appointment (which is a difficult feat itself where i live in the UK).   

Have been reading the forums and I see others having issues with Elleste duet but not much on Elleste solo 1mg, so looking for some advice or has anyone else had issues with the fatigue and muscle pain.  Only way I can describe it is that it feels like the flu without the other flu symptons.  Feel like I am going crazy  :(

Hi Scookie - welcome to the forum - lots of help and advice on here as well as friendship and encouragement through all that life can throw at us.

I wonder whether your HRT dose is too low due to your young age but someone will be along soon with more knowledge than me. I just wanted to welcome you really and let you know that you are not alone.

Have you checked out the advice under "Menopause" in the green bar at the top of the page? There are details there about premature menopause and early menopause.

They do (quite rightly) start you on low dose.  I started on 1mg elleste solo which worked but within a few weeks symptoms were creeping back in so I was increased to 2mg (max dose), as you're tolerating it I'd mention it is worth trying 2mg.

If you continue to experience symptoms then you could try a different delivery method eg patches or gel oestrogen, because some women absorb oestrogen poorly from tablets?  I agree with Taz that ultimately you would need a higher dose at your young age but as you have the Mirena it is difficult to know where you are in menopause. Also if you are still peri-menopausal then your own cycle may sometimes kick in. Mirena does not always prevent ovulation so if it does then you may well experience some hormone fluctuations.

Have you also had blood tests to check thyroid function and general health eg minerals such as iron? It is important at this stage in your life to make any lifestyle changes that might be necessary, and to ensure you have a really good healthy diet and plenty of exercise so that you are in the best of health as you approach menopause and beyond.

Hi I am also early menopause. 43 and has been started on 
Elleste Solo. I feel really tired all the time and no motovation. Have been losing weight for over a year and was getting on well. Recently no inclination to exercise and come home from work and just watch box sets. Feel the weight is going back on but not sure if it is the no exercise or the new tablet? Only been in it 3 weeks.  1st time ever on HRT and also have the mirena coil replaced after my 5 years up from previous coil. Anyone else have weight gain and no interest is work or exercise?

Hi Jak03 - sorry to hear this. Have these symptoms come on only since starting the Elleste or have they been coming on gradually? Oestrogen replacement - or any HRT needs to be given 3 months at least to assess its effectiveness. When did you have the Mirena replaced? If this was also  done at the same time you will be getting a much larger dose of the progestogen into your system than from the old one so your feelings of lethargy could be due to this - as the hormones is a sedative.

If you are only taking 1 mg you might like to increase to 2 mg if you are not experiencing relief from symptoms and are not feeling energised ( oestrogen should improve your feelings of well-being if it is working!).

Also the nearer to menopause you get the more exercise and less food you need to eat in order to stay the same weight ( ie avoid gaining weight) as our metabolism slows. Could this be happening with you? I struggle all the time to keep weight increase at bay and go to exercise classes, eat hardly any sugar or refined carbs etc - I'm in mid 60's.

Hope things start to improve soon.

Jak03, if I were you, I would immediately increase the dose to 2mg Ellesse and it that doesn't work, swap the tablets for Oestrogel and I will explain why.  Obviously this is just my own personal experience and opinion but (most importantly) I hope it helps you and others reading this.

I originally had a Mirena coil in 1998 for contraception but once my oestrogen started dropping drastically in 2003 (aged 42), I started to have real problems. 

The very first problem was what I thought to be thrush and cystitis but it was vaginal irritation and drying caused by lack of oestrogen and too much progesterone in the coil even though it was about to expire - I was told in 1998 that the Mirena had a 10 year lifespan.  The other problem that kicked off in 2004 was silent migraines also caused by too much synthetic progesterone and very low oestrogen.  It was at this point that I started sweating far more than before and it was horrid although to this day I have never had a hot flush or night sweats. 

I had various tests and my oestrogen levels were frequently rock bottom and FSH sky high so I immediately started on oestrogen patches (with the Mirena still in place) but they didn't work and I couldn't get anything like enough oestrogen out of them.  So I felt very much the way you do now, very flat and very low and wondering why none of the HRT I had tried was working.  In the end, I gave up on the coil and had it removed.  I had a year off HRT altogether (no periods so post menopause) but got sick of the sweating so I tried Angeliq (continuous combined form of HRT) and failed, tried Angeliq plus top up patches for extra oestrogen which was somewhat better but I still felt half backed, tried oestrogen patches (I got up to 100mcg) and Utrogestan and failed.  I would add that I was completely on my own with this because I had a useless GP so I had to work it all out for myself and buy the various types HRT (but not the Mirena of course) over the counter in Spanish pharmacies.

I finally reached breakthrough after wasting a lot of time on stuff that didn't work and I am now on the Oestrogel (oestrogen gel you rub on your skin) and Utrogestan (you won't need this if you have the Mirena coil) regime and it works extremely well.  I don't have any menopause symptoms or any of the problems I had before as long as I keep to a super low dose of Utrogestan.  I am now 56 and about 10 years post menopause.

As I regularly say on here, I wish I had tried Oestrogel with the Mirena coil but I had never heard of it until I consulted Professor Studd.  I think the gel would have worked better with the Mirena because it is flexible, you can easily adjust the dose up or down and it is easily absorbed.  The progesterone in the Mirena is very strong and it does get into your overall system so you will probably need quite a high dose of oestrogen to counter the side effects (low, flat mood) and frankly, 1mg of oestrogen is not enough. 

I hope that helps!  I can't guarantee that any of this will work but it has to be worth a try. 

I joined MM in the hope that my terrible experience and endless failures can help others fast track to something that works better.

Hi MaryG if you have the time can you explain how this gel works please. i had hysterectomy almost five years ago and I have tried hrt three times without success. How do you know how much gel to use? and how often. sorry if question seems silly but I am willing to try anything to help meno symptoms.

Yammy1, no problem, I'm always happy to help!

Oestrogel is an oestrogen gel that comes in a canister with a pump.  You just pump it out of the container and rub it on your skin and it takes a couple of minutes to dry.  The instructions say it can be applied either on your upper, outer arms or your thighs.  The lowest dose is 1 pump everyday and the highest is 4 pumps everyday.  I am on 2 pumps everyday and I split my dose by 12 hours to get an even dose.  It is trial and error as to how much you need but it is easily to adjust the dose to suit your needs.

If you have had a hysterectomy it should work a treat because you don't have to worry about the wretched progesterone.  I would start at 2 pumps and get a good dose of oestrogen into your system because of your night sweats and you may even need to go to 3 pumps.  You may also want to split the dose like I do to get an even spread.  Whether you use it morning, night or both like I do is entirely up to you. 

Why don't you ask your doctor to prescribe it?  The best approach is to be assertive and tell them you want Oestrogel.

Yammy, in the UK, a private prescription costs about £8.00 per canister of Oestrogel.  I don't know about anywhere else apart from Spain where a tube of Oestraclin (exactly the same as Oestrogel) costs just over €4.00 - you don't need a prescription there, you can buy it at any pharmacy.

I live in Ireland and I have to pay doc€50 just for visit, I have no idea what hrt will then cost. But I'm heading to Vigo Spain early September so I might be able to pick some up there if it's around €10 for 1 pump and I could need up to 4 A day that's €280 a week if I'm getting you right. No way I could afford it  :-\

Yammy, it costs €4.00 for the whole 80 gram tube not 1 pump.  Depending on how much you use, 1 tube will last about 2 or 3 weeks so not as expensive as you feared!

Can you get Oestrogel in Ireland?  Even if you can, it will probably be cheaper in Spain so my advice would be to find a pharmacy at the beginning of your holiday and place an order straightaway.  Work out how much many tubes you will need, order it and they should be able to get it in for the next day.  Spanish pharmacies usually only stock a couple of tubes but they have deliveries everyday and they also take credit cards if that helps.   

Don't forget, it's called Oestraclin in Spain and the staff don't always speak English so make sure you write it down.  It comes in a red and while box and looks like a toothpaste tube.  I've never had any problems getting it into the UK and I always carry some in hand luggage - it is under the 100 gram liquid restriction anyway.

I am going to put names in my phone so I will have it for doc and for Spain. Once again  thank you so much for your help, this hrt and menopause is a minefield

Yammy, you are more than welcome!  You could at least get a couple canisters of Oestrogel from your doctor in Ireland to get started and then buy the rest in Spain, it should work out cheaper.  It certainly is a minefield and it takes ages to learn about the different types of HRT.  Good luck and do let us know how you get on.
