POST MENOPAUSAL BLEEDING & MIRENA

I'm 54 and have been on HRT (Evorel Conti) for just over a year.  A few weeks ago I started bleeding, I went to see my doctor immediately and was referred to a consultant (luckily I have private health insurance).  Initial examination suggested that it was unlikely to be anything serious, however I have been booked in for a hysteroscopy under general anaesthetic.  At the same time she wants to insert a Mirena coil.  Does anybody have any experience of this in these type of circumstances? 

In the meantime, to stop the bleeding, she prescribed progesterone (Norethisterone).  I've been taking this for about 3 weeks and I'm getting really bad cramps - not sure if it is the medication, or just a coincidence and part of the original problem.  I'm still getting some bleeding, but I'm loathe to up the dose as that seems to make the cramping worse (she said I could increase the dose if needed).  At the moment I'm tempted to stop the Norethisterone and put up with the bleeding, at least for a few days to see if the cramping gets better. Unfortunately the consultant is away at the moment so I'm unable to talk to her.

Any help, advice or general information would be really great.

Were you post-menopausal when you started the Evorel conti ie had you been without natural periods for at least 12 months before starting the Evorel conti, and had they been very infrequent before that? If you are not post-meno then this could explain the bleeding ie breakthrough hormones/cycle of your own.

Also did you have any bleeding in the initial weeks or months of this HRT? It may be that the balance is not right for you or perhaps have a fibroid or polyp which could cause the bleeding? Anyhow at least you are being investigated and hopefully all will be well.

Many women use the Mirena very successfully to prevent heavy bleeding on HRT and provided that any pathological reason for the bleeding has been eliminated, it sounds like a good idea if you are happy with it. Then you would be able to add oestrogen at the same dose as before (50 mcg) or slightly higher if necessary. It would keep the uterus lining thin and hopefully prevent further bleeding.
