What to ask my GP to prescribe with utrogestan? Oestrogel?
« on: September 04, 2016, 08:07:12 PM »

So I am doing my homework prior to asking my GP for a new prescription having tried elleste duet conti and not liking it. Am I right in thinking utrogestan is the only bioidential progesterone? And what form of oestrodiol should I be asking for, to take alongside it? I have seen on Prof Stubbs' website that  recommends oestrogel & utrogestan. I am 20 months after my last period so happy not to bleed though to be honest would absolutely take a scanty bleed if I have to. Not sure how the utrogestan works in that respect.

Elleste has the most powerful progesterone which many find brings PMT type symptoms.  Femoston is another pill option as the progesterone is kinder. Have a look under TREATMENTS at the top of this page. If you want to try Utrogestan, print out the section on Progesterones to show the GP.
Utrogestan is usually under ‘micronised progesterone in their drugs book.  Utrogestan is bio identical but doesn't suit everyone - it is advisable to try the sequential regime first to see how you tolerate the Utrogestan, as taken continuously can make one feel rather sedated. We are all different and the bleeds with Utrogestan can vary greatly - some problematic bleeding and spotting is common with Utro.

Oestrogel allows flexibility with dosage but some don't like having to apply it every day. Patches are popular as an alternative. Look under Oestrogens to see your options.

If you really want a convenient non bleed regime then it's worth considering having a Mirena as this generally gives fewer progesterone side effects and allows you to use as little or as much oestrogen as you need.  Initial side effects and bleeding usually subsides within 2-6 months with the Mirena.

All HRTs need a 3-6 month settling time but it is trial and error.  Good luck DG x

Thank you DG! I think a Mirena is out for me as my vaginal atrophy is so bad at the moment that I had to abandon my smear test last week and GP said she didn't think she would be able to get a Mirena in! I have been poring over the treatment pages overnight (managed to sleep from midnight till 4am and that was it 😏) and will come to a decision. I am leaning towards trying the fem seven conti patches at the moment thinking if I don't get on OK with those I would then try the oestrogen and utrogestan. When you say the progesterone in femoston is kinder how does that compare do you know with the progesterone in femseven conti which is levonorgestrel? thanks again

Femseven has a fairly strong synthetic progesterone that is still different to Norethisterone in Elleste - however both these progesterones are more testosterone based.  The progesterone in Femoston is less testosterone based and is therefore tolerated better.  Any progesterone taken continuously will often some side effects. Do look at the info on this page from this site which explains about the difference:

In your situation I would be more inclined to try the Femoston 1/10 sequi to see how you feel on that progesterone.
If your VA is really bad then you really must ask for some Vagifem to use alongside full HRT - you may well need both and then you would have the option of having a Mirena if you wanted a no hassle non bleed HRt combination.
