Evorel Sequi End of Month - Technical Advice please?
« on: June 14, 2016, 06:38:56 PM »

Hello

I am nearing the end of my first month of evorel sequi and need some advice please?

When I get to the end of the month, provided I don't get any adverse reactions to the progesterone, I'll be able to assume that I am not progesterone intolerant won't I?

When my first period comes and if it's light, will I be able to assume that my oestrogen levels are still quite low? Been taking a form of oestrogen for 2 months now.

As I am taking HRT for mood issues primarily, should I be aiming for a measure of 600/800 pmol of oestrogen as Prof Studd mentions? Again this was measured at 127 pmol in April.

My gyny that I saw last night wants to see me again in 2 months time after I've finished a full 3 months and says she may move me to a continuous regime later on but not sure why.

Any ideas anyone?

I think you need to give it three or four months to see whether you are OK on the Evorel or not

Hi - thanks for the reply.

I was hoping that I am not progesterone intolerant because my gyny maybe suggesting the mirena in a few months time.  Also I suppose I am preparing myself to drop at some point....hopefully it won't happen.  Strangely though, I'm sure that I started to bleed last night but it's seems to have stopped again now....is that normal do you think?

Hi Mis71Mum

If you are feeling OK on the combi/conti part of the HRT then sounds like you are OK with this progestogen. You will expect to feel normal pmt when you change back to the oestrogen only patches as this is what happens during a normal period before the bleed. After a few days you should feel good again :)

If your period is light it won't reflect your oestrogen levels particularly - just that your womb lining hasn't built up - because the progestogen is doing its job keeping it thin while you are taking it, and/or the oestrogen level is low. Also absolute oestrogen levels are not necessarily indicative of mood - I wouldn't get hung up on needing to attain a particular level - but just a dose that makes you feel OK.

Having said that 50mcg patch is not a high dose, although if you are peri and still have quite a strong cycle - then during the oestrogen peaks mid cycle your levels may well be fine. If the aim is for these levels to be high during the whole cycle I doubt whether this would be achieved with a 50 mcg patch. However you may feel well on this dose or even a small increase in patch size?

If you are peri then breakthrough/withdrawal bleeding can sometimes occur randomly due to your own hormonal fluctuations .

Hi Hurdity

Many thanks for the reply. Over the last 20 days, I've only had 4 down days which is an improvement on previous months. However I'm beginning to feel a bit lower today but my period does seem to have started today.

I was a bit confused as my gyny said that the patches only come in 50 mg but was sure people on here have mentioned 75 mg before.
However she wanted me on a continuous steady dose before making any changes.  She did mention that I could switch over to mirena and oestrogel maybe later but a really daft question, do you still get periods?

The Evorel sequi patches only come in 50 mcg - as part of the sequi regime.   Evorel oestrogen patches come in higher doses but then you would need a separate progestogen or a Mirena coil. They are all listed here (the oestrogen preps) http://www.menopausematters.co.uk/treatafter.php

Re the Mirena and periods - many women stop periods completely on t he Mirena even if nowhere near menopause so if you are well into peri then this may well happen with you - although women report some bleeding for the first few months until it settles. As you have been on HRT maybe this won't happen?
