49, perimenopausal and I've used HRT for about 3 months now.   Vivelle 0.0375 patch and 200 mg Prometrium for 12 days only every 6 weeks.  My practitioner agreed to this and feels it is fine (progesterone does not always agree with me), so we decided on this particular regimen.   Had a very light period last time I took the 12 days of Prometrium.   This time, I am 10 days out from the last 200mg pill and nothing.   Could it be that my estrogen level is too low for a proper shedding?   Had a couple of days of very light pink/brown spotting.  Not even enough to use a pad.   Any thoughts are welcome.

This happens to me too. I'm 49 and using 37.5 micro gram Estradot (I think this is the UK version of your patch) and 10mg Provera for 12 days. Sometimes I have a bleed and sometimes not. The spotting was probably your bleed. There's nothing wrong with this but if you need reassurance you should speak to your practitioner

Cyclical preps give a monthly bleed in about 85% of women so you and I might fall into the 15% who do not bleed.

  from me too. I guess you are across the pond?

I tend to agree with Galadriel - that your oestrogen dose is quite low which may be why you aren't getting a bleed. I use 50 mcg of the same type of patch but use 200 mg utrogestan vaginally now every 2 months - but I always got a bleed even when having the utro every 6 weeks. Are you taking it vaginally or orally? Even if it wasn't sufficient to shed all the lining - if the lining had built up some of it would be likely to come away after 12 days prometrium.

Thank you both so much for your replies. Yes I'm in the U.S. Using  the Prometrium orally.  From what I've  read, it's not the strongest or most potent gestagen so perhaps that, coupled with the low estradiol patch made  me skip the period.  I have read of others not getting a period after the progesterone part of the cycle and was just curious what your experiences were.  We'll see how the next cycle goes. I appreciate your responses!
