HRT message for Hurdity

Hi Hurdity, can you advice me as you have helped in the past. I am 63 post menopause for 10 years. Was put on Evoril contii and was ok for a few years but then started with the bleeds. Gone said it was the HRT and needed to come off so am now feeling low and tired with night sweats. He has suggested estrogen by mouth and a merina coil fitted so I have localised progesterone. He didn't give me any other alternatives. Do you think I should try this? Can I not just have estrogen only?

One needs progesterone in order to keep the womb lining thin enough to be healthy.  There are several ways of doing this, hopefully you will get more info. as the day goes on.

How long did you stick with the conti. It took me a good six months before my bleeds eventually stopped. If Evorel otherwise suits you then you could use a sequi regime then you would at least know when you would have a bleed. Although in your 60s I'm sure you would rather have no bleeds at all.
Progesterone is always needed if you have a womb but you could also consider a very long cycle.

Have you asked your GP for a referral.

Nice of you to think of me but there are lots of others on here who have the same knowledge about HRT so do post a general question and you will be more likely to get a reply especially as I'm off on hols tomorrow!- if you want to talk to me personally you can pm me!

Anyway re your Q - the main thing is were you investiagted re the bleeds on Evorel conti and how long after being on it did the bleeds start? I presume you were properly post-meno when you started the Evorel conti ie no period for at least a year?

If there is no pathological reason for the bleeds then it could mean that the balance between the oestrogen and progestogen was wrong. As CLKD says you can't have oestrogen only if you have a womb - you have to have a progestogen. However there are lots of alternatives!

Yes you can have a Mirena - but you don't have to have tablet oestrogen with that in fact it is not advised for over 60's to start taking tablet HRT. Far better to use transdermal - - patch or gel. If it were me, I would go for a patch usually changed twice a week (I use Estradot which are very small). Others prefer using gel which is applied daily. I can't believe the gynae didn't suggest anything else and also v surprised he recommended a tablet if he is a specialist!

Hi Hurdiity, yes has a biopsy for the bleeds, all ok. Was on it for about 3 years. Was properly. Post meno 10 years on. Will have a good think about it and discuss with my GP. Thank you so much for your help!

Hi charliegirl - I agree with Hurdity that tablet HRT is definitely not advised for over 60's due to the increased stroke risk. Transdermal patches or gel is thought to be safer.
