Yes, I rarely have daytime flushes but often have night sweats (I'm not on HRT though).

Hi vanessa spain - just wondering about your HRT - and what preparation you are using? Progesterone cream (that you buy online and which comes from US) does not contain sufficient hormone to protect the uterus lining from stimluation from oestrogen in HRT. Maybe you are using something different. I know that in other countries some women pay (a lot of money!) privately to get a specially compounded progesterone cream that is designed to give the correct dose.

Some progestogens seem to have the effect of increasing flushes maybe because they bind to oestrogen receptors and interfere with the beneficial effects. Do you apply the progesterone at night and maybe this preparation is not right for you? Alternatively it could just be that that is how your body's temperature regulation is affected.

The other reason could be the oestrogen dose is a little to low - ideally your HRT should eliminate sweats and flushes. When I reduced my dose last year down to 37.5 mcg patch from 50 mcg I notuiced the first thing to come back were the night sweats and I was beginning to have disturbed nights in the latter half of the night. These disappeared when I went back up to the 50 mcg dose.

Perhaps you can let us know what doses of each you are taking and when.

I had night sweats for 10+ years on combined oral contraceptive pill.  I have read that it may be the progestogen, as it is heat generating. (When charting menstrual cycles, ovulation comes with a spike of progesterone and a temperature rise).  However, mine went away on the progesterone only pill.

I have not had them whilst on nothing, nor on hrt.  Apart from 3 nights a couple of weeks ago that I cannot explain. :(

Do you have progesterone in your hrt as well?

Hi Stellajane

were having 2 pumps in the morning?
If you were then you could try taking 1 in the morning, then 1 later in the day?

If you spread out the intake then the pain might subside.
This would help with the overnight problems too.

I find night flushes disturb my sleep even though they are not severe because I am quite a light sleeper. Mine seem to happen in cycles and I can go a few weeks without them then back they come, little blighters.

Hi vanessa spain

OK I've looked up Perifem and looks like it contains 2 mg Estradiol valerate and 10 mg Medroxyprogesterone acetate  (MPA  the progestogen) which doesn't have an equivalent for peri-menopause in UK.

The first thing is  - if that is what your tablets contain, then it is a complete HRT preparation in itself and you do not need the progesterone cream. If you are still having flushes on this preparation then I would suggest the dose of HRT is not right for you.

I don't know where you are in menopause and how far into it you were when you started HRT and how regular your periods were, but it could be you need a higher dose. Maybe you are not absorbing sufficient of the oestrogen from the tablets - which does happen with some women. As your flushes are not being controlled this is most likely the explanation.

I would completely stop the progesterone, and perhaps think about a transdermal form of hRT ie patches or gel (and separate progestogen - not progesterone cream). I've just looked up Darstin though and looks to be a progesterone gel like Crinone - if so then it can be used together with oestrogen to protect the womb but you wouldn't need to have HRT that contained progestogen if the dose used is sufficient.

It's difficult to advise as we don't have these products but the main thing is I think you need more oestrogen, and less progesterone - unless the gel has been prescribed in addition to the HRT tablet to deal with heavy bleeding.

Apologies if I sound confused, and that's because both the products and dosage/prescirption must be quite different in Spain. The biological principles are still the same though!
