polyps, biopsies and Mirena
« on: October 03, 2014, 08:53:04 AM »

Hi all,

Sorry if this is long. Started HRT back in March on Evorel Sequi. Was still having periods but very irregular. Symptoms improved dramatically, except I was bleeding every couple of weeks. Had cervical screening, and a polyp was discovered on my cervix. I've since had ultrasound scans, both internally and externally, to see if there were any internal polyps, and to see if there were any issues with my ovaries. All was fine, and the polyp is just on my cervix, although on my doc appt today the doc did say my womb lining was around 5mm thick. She said this would be an issue if I wasn't having periods, but as I'm still having bleeds, it is probably normal. She's now referred me to gynae, to have the polyp removed, and at the same time to have a biopsy of the womb lining - I think she called it a hystoscopy?  Still unsure whether the irregular bleedng is due to the HRT or due to the polyp.

My doc suggested that if everything was OK with the womb lining, that perhaps we might try the Mirena coil, along with oestrogen in pill form, as Mirena tends to thin the womb lining a bit, so helps to reduce heaviness of periods. She did had first suggested  coming off HRT altogether, but I'm really not ready for the sleepless nights, joint pain, flushes and all the other symptoms to return. She realised that my quality of life had improved so much, so that's why she then suggested Mirena plus oestrogen.

I'm really pleased with my doctor's actions, she seems very thorough. It's been a bit of a medical week for me, been either at the doctors or hospital every day (had breast screening too). The Pavlovian response is starting to kick in now, as every time I step into surgery I either want to whip my top off or drop my pants and open my legs LOL! ;D

Anyway, hope you're all coping, and if anyone has had experience similar to mine, or any suggestions or comments on any of this, I'd love to hear from you.

Thanks for reading

Good to hear your doctor is helpful and thorough - great to have some good news amongst all the tales of woe from others' encounters with some unhelpful and ill-informed GPs!

It's a hysteroscopy you have been referred for - if you do a search there are several threads on other women's recent experiences which are very variable. I had one of these last year to have a look at the womb lining due to post-coital spotting - and because an abnormality was found on the scan. They did a biopsy at the same time. If they had found a polyp they would have removed this at the same time - fortunately not. The abnormality was a small fibroid which is fine. I had mine done with no anaesthetic and it was all over very quickly with only minor discomfort, but I imagine you might be offered some form of anaesthetic if you are having the polyp removed - although this isn't always the case.

I'm not sure why the doc suggested HRT in pill form along with the Mirena? You could just as easily stay on Evorel - but instead of Evorel sequi it would be an oestrogen only patch Evorel 50 or another of the brands. If this has suited you apart from the bleeding I would stick with it ie Evorel following the Mirena. Personally I would not go from patch to a pill oestrogen.

I have no experience of Mirena but there are several on here who have and hopefully someone will share this with you!

I agree with Hurdity - if patches have suited you then I'd go for the oestrogen only patches with the Mirena. Another option would be to use Oestrogel which allows you to adjust your oestrogen dose to your needs.  I had the Mirena for over 4 years and it was the best option for me at the time.
Good to hear about your positive experience.  Good luck  DGx
