Still having problems what to do
« on: January 24, 2014, 10:46:18 AM »

Last time I posted here was August last year (http://www.menopausematters.co.uk/forum/index.php/topic,22564.msg342357.html#msg342357)

Went back to see my Dr after starting the above thread....she  is utterly useless....first of all when I suggested going down the HRT route to her a few months before as I was having such problems with joint pain/insomnia/over active bladder....she put me on  Evorel conti something I shouldn't have been on as I was still having periods....anyway I had to stop them because of palpitations etc......next she prescribed  Premique....again wrong medication as no break for me to have my period......I took them as prescribed and they did nothing for my symptoms  other than stopping my periods all together........I should perhaps mention at this point that I don't have hot flushes, night sweats or  mood swings as I take a high dose Red Clover tablet every day which works for me......When she gave me the Premique she told me to stop the Red Clover as the HRT would take care of everything......wrong....one week after stopping red clover the flushes night sweats started again........anyway eventually as the whole purpose for me taking HRT was to get back into a normal sleeping pattern....when that didn't work I went back to see her and ask to swap to Utrogestan ......she decided that she would take me off it completely as according to her there is no clinical evidence that HRT works for insomnia or joint pain......this was back in Sept.......I have been without anything since then despite 2 further visits re my insomnia she refuses to do anything about it.......I'm at my wits end now surviving on anything from 9 hours to 16 hours sleep per WEEK.......I'm constantly up and down all night because as I'm awake I'm more aware of my bladder urgency....also my RLS has returned...something I've not had for years but I'm sure its linked in some way......I'm going to try one more time to get her to see that I need help and it is hormone based as OTC meds have done nothing to help....I'm convinced that if I find the right HRT for me then most of my problems will go away........any suggestions please as to what I should ask for
If she refuses to help this time then I'm changing my surgery as I simply cannot go on like this....I almost had a car accident last week due to the fact that I was so exhausted

Hi tangobunny,

Sorry you are having such a bad time. Lack of sleep is just awful - I barely coped when it was happening to me. Has your dr offered anything for your sleep/bladder problems?
I've been on Femseven Sequi for nearly three months. Started on half a patch and felt fine, but felt worse when I increased to a full one so went back to half. Sleeping much better and most other symptoms gone - cheaper too as prescription will last longer using half ;)

Really hope your dr can give you some help :)

Hi tangobunny
You sound so stressed and exhausted  - you poor thing.
Sleep disorders are very difficult to treat - the problem is, it becomes a vicious circle.  The more difficulty you have sleeping the less you want to do during the day, you can get depressed and anxious and, I bet, you dread going to bed.
I go for a pee at least twice every night and often can't get back to sleep - it's torture. Dark thoughts plague me during the small hours and lack of sleep gives me headaches during the day.
If you are not getting night sweats and other meno symptoms, to take HRT just to help you sleep seems a bit odd. Yes, I do sleep better when on HRT but not always.  I think pinning your hopes on HRT solving everything could lead to disappointment.
I think it's about breaking the cycle.  I saw a programme that came up with a regime to help insomniacs.  It involved going to bed quite late and getting up quite early - absolutely no TV or stimulants in the bedroom. You then had to get up quite early regardless of how you slept and get through next day without any rests - somehow!!!!  Exercise and good diet was important e.g. walking and excluding stimulants like coffee etc. They were so exhausted they dropped asleep more easily and found it really hard to get up early the next day - it was important to keep this regime up for at least a week or so. The people on this trial regime quite quickly found they started to sleep better.
I have devised my own regime, I have a hot shower, read for about 20mins and I usually now fall asleep quite easily.  If I can't get to sleep when I've been up to have a pee, I just lie really still, do some relaxation exercises(self hypnosis technique) and try very hard to concentrate on something lovely or very boring!.
I also take a Nytol for a couple of nights if I've had a series of bad nights and it usually helps to break the cycle - you've said you have tried OTC stuff so I presume this is no good for you?
Aching joints -  I'm an ex dancer with dreadful joints and you've got to keep moving (walking or swimming is the best), lots of Omega oils and I've recently found that Glucosmine (1500 dose daily) is really helping.  Omega oils have been shown to help children with sleep as it has a positive effect on brain function - worth thinking about?
If you still want to try an HRT, why not Femiston Sequi? I know a few friends on this one and they are doing really well - it comes in different strengths as well so you could try a low dose to start with. Oestrogel or patch with Utrogestan is another option.
I hope I haven't told you things you already know or have tried - I  hope there is something there to help you.   I really empathise - DG x

Have you had any drugs to treat your bladder urgency? Have you had a diagnosis of what sort of bladder issues you have?

Something like Atarax ( hydroxyzine) can calm the bladder if there is inflammation and it's also a sedative- you only need take 10mgs or 25 mgs at bedtime. You might also get some improvement with the urgency by using vaginal HRT even if you don't think you have dryness.

Hi tangobunny

Sorry to hear you are still having problems.

I wondered what your periods are like at the moment ie how often you have them and how much space in between?

Are you waking because of the bladder urgency or over-heating due to meno-pausal symptoms?

Have you thought of using a vaginal oestrogen in addition to the re bladder irritation and sleep suggestions other members have made? Having said that I find that whenever I put something up there (Vagifem, progesterone, estriol pessaries) it causes me to have to get up in the night to pee but in your case as this is very frequent it could help?

I am similar to Dancing Girl in that once I've got up I can't get back to sleep but I posted on another thread recently about the 3-2-1 anti-anxiety exercise another member posted, which I adapted to help me get back to sleep and it does work ( sometimes!). It usually works better for me when trying to get to sleep when I first go to bed.
