Well I'm feeling a bit like I am back at square one again....

I'm 49, Peri, been on 3 pumps of Oestrogel daily with 14 nights of 200mg oral Utrogestan.

Started this regime at the start of 2021 to replace Femoston 2/10.  Took a while to settle on it but definitely had a good run of months where I felt it was working well.

However over the Xmas period symptoms have started to creep back in...

Fatigue, headaches, migraines, hot (but not necessarily hot flushes), sleeping well but feeling just as exhausted on waking, fizzy anxious chest on waking and periodically throughout the day, aches and pains mainly in hips, left ribs and hands, palpitations...

I am under quite a bit of stress at the moment, well its been a couple of months really and wonder if this has brought things on or whether I need to reassess my dosage etc?

Any feedback would be great.

Hello,

I know it was several months ago that you posted but was wondering how you got on? I seem to be in the same place at the moment. Was doing really well on hrt but palpitations, chest pain and anxiety are back with a bang and I feel as though I’m back at the beginning again.
I had an ecg and bloods last week but so far all seems to be ok.

Hi it sounds like you need more oestrogen if symptoms have returned. Blood tests are unreliable and not needed over the age of 45.

Thanks for your reply.

I wondered if it was due to hrt, oestrogen levels. Trouble is peri has triggered health anxiety so i find it hard to think rationally. I have been on 2 pumps a day for over a year now..

The bloods I had were just general fbc, ferritin levels etc as gp wanted to make sure they were all ok.

Hi have no womb but have estrogen dominance. I went to a private clinic and was prescribed 100mf Utrogestan. To take every other day for a week then every day. Increase if needs be. The sensitive effect was great as I wasn’t sleeping. It also helped with the severe diarrhoea  but as soon as I went to every night a got really bad depression and really bad thoughts all the following day till around 10 on the evening.  That evening I didn’t take one and the following day was a good day. Fairly cheerful and wanting to do things and o was me again till the early evening when I think my continuous estrogen dominance anxiety etc comes back.Tried again to see what happened  same thing.  Spoke with clinic during one of these depression episodes to another GP she is suggesting reducing estrogen doesn’t sound like she believes in estrogen dominance ( it worries me that GP from same well known clinic have varying beliefs ) and trying histamine diet also has put me androgen. I have tried testosterone previously and after 2 months I had an itchy skin and a small number of  small white blebs and small number of little red spots. Blood tests showed an tiny increase in red blood count that none of their GPs are worried about.  I am!  I am really really scared now that I’m not going to find a way through this. My estrogen anxiety is through the roof all day and night.  Joint pain back pain really dry skin. I’m just asking a question again I think. If I took Utrogestan vaginally would the dose be the same. It just seem weird that the following day I was cheerful. Thought if perhaps there’s a way of getting a lower dose I could perhaps tolerate it.  I suspect not. Bad thoughts again 😭

Original meno symptoms were severe muscle spasms at or when trying to relax. Made my whole body jolt. Twitchy muscles arms n legs jumping.  Not restless legs!! These have gone with Estradot 100.  You can see perhaps why I don’t want to reduce my latch as they do come back at 75mg. Even if I cut around a fifth off the patch I get littl spasms

Can someone explain my body. I don't understand it myself.
I take one sachet of Sandrena gel and one Utrogestan pill continually but still get intermittent bleeds that last the odd day or two and a few days before get breast tenderness which stops shortly after I bleed. I thought if I took both pills continually I wouldn't bleed.
I've tried coming off them completely but the reasons I take them in the first place come flooding back.
I am 12 years post menopause so shouldn't have much in the way of my own hormones left.

It does help thank you. I would have thought the most sensible way of taking them would have been to take the Sandrena gel for the first 14 days and just the utrogesterone for the following 14 days. Therefore creating a more 'natural' cycle. I haven't been brave enough to try it even so.

It's the oestrogen that controls your symptoms so if you stop it for a fortnight they are likely to return. You could try using utro vaginally as more will get to the womb that way, or doubling the dose or trying a different progestin such as provera. A mirena is the best at sorting out problem bleeding. Or you could try using utro with a 3 day break per cycle. It's usually used in late peri but allows a bleed if there's been a build up.

Just after some thoughts. At a gynae appointment on Monday I was advised to carry on with 100mg Utrogestan and estrogel regime but to possibly try the Utrogestan vaginally.
Bit uncertain whether to use this continuous for age 56 and the odd period now and then still or for a break of vaginally?

Another option would be to use it for 25 days out of 28. This can be useful in late peri as it allows a bleed if there's been a build up.
Using a sequi or conti regime now is probably personal preference, if you want a predictable bleed use sequi, if you'd rather not have a monthly bleed use conti and accept there will be breakthrough bleeding from time to time.

Yes I like the sound of using it 25/28 days as I am definitely in late peri.  I don't really want a monthly bleed at all, but happy to have the odd one now and then!!  Hopefully not much more of this now at 56... :o

I’ve been on 3 pumps Oestrogel and cyclical Utrogestan. Now on cycle 23. Symptoms began to return: cyclical depression (occasionally), recurring UTI, vaginal dryness, insomnia, tearfulness, high anxiety, night sweats and (not sure if HRT related as I’ve had migraines for many years) awful migraines, including a recent hemiplegic one and continuous one for 4 weeks.

GP upped Oestrogel to 4 pumps and suggested I switch to continuous Utrogestan at 100mg per night.  I didn’t bleed last cycle and I’m not sure when to begin continuous, I’m currently on night 5 of Utrogestan.

I asked for referral to menopause clinic but she said no need to because she can try different oral HRT. I told her I have migraines with aura so need to stay on transdermal. I was offered antidepressants, I’d rather see what I’m like on right amount of HRT because it was controlled when first started it.

Any thoughts?

There's a good chance 4 pumps will cure your symptoms so I'd stick to this for the next 3 months to see. Perhaps see if you bleed this time, have a break for 5 days then continue on 100mg conti. You could use it for 28/28 to start with as this allowsa bleed if there'sbedn a build up. You may find you need topical oestrogen for VA too, hrt doesn't always control it.

I really hope the increase does sort out the symptoms.  I’ll do that.

Could I ask what 28/28 means, sorry I’ve not got a clear head.

I’ll see if systemic increases resolves dryness and VA and if not I’ll request topical.
